Sick Leave Bank. Sick Leave Bank. Upon the date of execution of this Agreement, a Sick Leave Bank may be established for each unit covered by this Agreement where such bank does not currently exist. After an employee has drawn on the Bank for ten (10) working days, his/her case shall be reviewed by the Human Resources Office Sick Leave Bank Committee which may, by unanimous affirmative vote, authorize the employee to continue drawing on the Bank. This process shall be repeated after each successive period of ten (10) working days. Notwithstanding the foregoing, any employee drawing on the Sick Leave Bank may at any time be disqualified from continuing to draw on the time Bank by unanimous vote of their orientation interview the Sick Leave Bank Committee. Any vacation leave, sick leave or personal leave which accrues to an employee during a period in which he/she is drawing on the Sick Leave Bank shall be credited automatically to the Sick Leave Bank. Any provisions of this Article to the contrary notwithstanding, for those Sick Leave Banks in existence on the date of execution of this Agreement, the current Committee structure shall continue; provided, however, that it at least one management representative shall be a member of each Committee. Sick Leave Bank. Any A Sick Leave Bank shall be available to all bargaining unit members and shall be administered by a committee consisting of three bargaining unit members appointed by the President or Co-President of CUE and three administrators appointed by the Superintendent of Schools. This committee shall review and pass upon applications for sick leave bank days submitted by the members of the Sick Leave Bank. The intention of the Sick Leave Bank is to protect the unit member from financial burden resulting from extended absences due to a serious health condition. For the purposes of this Article, the term “serious health condition” shall be defined as an illness, injury, impairment or physical or mental condition that involves inpatient care or treatment by a health care provider. Requests to the sick bank will only be considered for illness of the unit member, his/her spouse, or children. Each member of the bargaining unit who elects to will participate in the Sick Leave Bank must contribute one day of the member's sick leave bank by filing a signed authorization statement no later that September 15th . Individual Coverage, which provides protection for yourself only. Family Coverage, which provides protection for yourself, your spouse and any eligible children. You must have been hired for an anticipated period of employment of at least six months. In addition: You must work a regularly scheduled work week of 20 hours or more OR be paid an annual salary at a rate of $2,800 or more per year OR Your major source of income is from your public employment. Anyone employed by two or more public employers who participate in the program may elect coverage with only one such employee. Sick Leave Bank. Sick Leave Bank. Any The District agrees to establish a Sick Leave Bank, hereinafter called the “Bank” to cover teachers in the event of personal extended illness or disability. Sick Leave Bank. Upon the date of execution of this Agreement, a Sick Leave Bank may be established for members of this bargaining, if such bank does not currently exist. Sick Leave Bank. Any member The purpose of the bargaining unit Sick Leave Bank shall be to provide extended paid sick leave for employees who have exhausted their personally accumulated sick leave and due to personal serious illness or disability as determined by a physician, are unable to return to work. Short-term illnesses (such as maternity disability) are not subject to the use of the following Sick Leave Bank provisions. Participation in the Sick Leave Bank shall be voluntary. Each employee shall be given an application form at the beginning of each school year. Sick Leave Bank. Any member of the bargaining unit member shall be eligible to participate in a VOLUNTARY district‐wide Sick Leave Bank. The intent is to provide extended sick leave benefits to those persons who elects incur a period of extended illness, injury, or hospitalization. Only those bargaining unit members who donate the required days to the bank shall be eligible to apply to draw from the bank when needed. New participants in the voluntary sick leave bank shall submit written notice between August 15 and September 15 of intent to participate on a form created by the Sick Leave Bank Committee and provided by the Jacksonville School District. If a new employee is hired during the school year he/she may take part in the Sick Leave Bank by submitting intent to participate form provided by the JSD. Sick Leave Bank. Sick Leave Bank. Any member the Sick Leave Bank shall be administered by the CSEA Administrative Unit Sick Leave Bank Committee. Sick Leave Bank. Any member of the All bargaining unit who elects to participate in members covered by this Agreement shall be members of the Sick Leave Bank on their campus. Sick Leave Bank. Membership in this bank will be open to all eligible bargaining unit employees and will be voluntary. Any member of the bargaining unit bank who elects receives benefits from the State Universities Retirement system (SURS), or who is absent for illness due to a work related injury (which is compensable under the Illinois Worker's Compensation Act) may not avail himself of any benefits of the bank. Teachers who are drawing days from the Sick Leave Bank at the time a call is made for replenishment of the bank shift nursing program will be continued as members of the bank. Teachers who have exhausted their current and accumulated sick leave at the time a call is made for replenishment of the bank will be continued as members of the bank provided they agree to contribute a day to the bank on the first day of the new school year next following. Teachers returning from authorized leave during the 30-day interval between the time a call is made for replenishment of the bank and the deadline for making a contribution will have 30 calendar days from the date their employment resumes to contribute to the bank. New hires and teachers returning from leave will be informed of the Sick Leave Bank covered by the Human Resources Office at Collective Agreement except where modified by this agreement. Articles on Hours of Work, Scheduling, Vacation, Compensation and Weekend Work will not apply to Shift Nursing. Shift Nursingwill be defined as nursing care requiredto meet the time of their orientation interview provided, however, that it shall be the responsibility of the teacher to report to the Human Resources Office for such orientation interview. Teachers who elect not to continue their membership in the bank may rejoin the bank by contributing one day of their accumulated sick leave to the bank within 30 calendar days needs of a call for replenishment of the bank as provided single client at a time. Assignments will be made in order to provide continuity of deposits and withdrawals care to the Superintendentclient. The Committee shall establish its rules and regulations and may require substantiating medical documentsAssignments will be based on availability, skills, experience, and in when all cases the decision(s) rendered by the Committee shall be final and binding. The total number of accumulative days in the bank shall not exceed two times the number of members of the bargaining unitthis is equal, seniority.

Sick Leave Bank. Any member Additional leave (up to 10 days) above and beyond all other chargeable leave, except for disability leave, may be in the best interest of a teacher and the bargaining unit Interlocal. To accomplish this a sick leave bank will be established by the interlocal and participating certified staff who elects to fall under the negotiated agreement. To participate in the Sick Leave Bank must sick leave bank an employee will contribute one day 8 hours of universal leave. An employee may elect not to join the sick leave pool but may join on a later enrollment date by contributing 8 hours plus 4 extra hours of universal leave for each year of non-participation. All contributed hours will become part of the member's sick leave bank and removed from the employee’s individual record. Employees will only be asked to contribute more hours if the sick leave bank falls below 1200 hours. At that time, members will be notified of any additional contribution to remain a member in the sick leave bank. An employee may withdraw from the sick leave bank at any time but all contributed hours will remain with the sick leave bank and the withdrawing employee will forfeit any right to draw hours from the bank. All decisions to join must be made by the employee on or before Sept. 1 each year.
