SERVICES TO BE PROVIDED BY THE CONTRACTOR. The CONTRACTOR shall provide to the DISTRICT on the terms set forth herein all the services articulated in the CONTRACTOR’s proposal which is attached hereto and incorporated herein as EXHIBIT “A” (the “CONTRACTOR’s WORK PLAN”). In the event of a discrepancy, inconsistency, or other difference between the terms of the CONTRACTOR’s WORK PLAN with this AGREEMENT, the PARTIES agree that the terms of this AGREEMENT shall govern and be controlling.
SERVICES TO BE PROVIDED BY THE CONTRACTOR. As the Building Inspector 1. Contractor will review and issue a building permit for: a.) all buildings and additions thereto; b.) wrecking or razing, c.) moving of a building, and/or d.) occupancy of new structures or changes of use, consistent with provision of the Village’s Municipal Code, Chapter 14. In providing such services, Contractor will ensure that all permits are complete and accurate prior to issuance. All approved permits will be delivered or mailed to the property owner within one business day after said permit is approved. The Contractor may give the property owner verbal approval to precede with construction, however, this does not release the Contractor from his responsibility of delivering the approved permit to the property owner in a timely manner as defined above. 2. The Contractor will conduct inspections for all building permits issued. The number of inspections for each building permit issued will be determined by the Contractor based on the nature of the project in question and in conformity with acceptable building inspection standards, e.g., new home construction may require 6 to 9 inspection visits whereas a small project, such as a deck, may require only one or two visits by the Contractor. 3. As needed, the Contractor will consult with the requestor of a building permit to resolve any conflicts and obtain any information needed prior to the issuance of a building permit. The Contractor is expected to take all reasonable actions needed prior to issuance of the permit to ensure that the proposed construction meets all applicable building codes. 4. The Contractor will maintain a written, up-to-date record of all building permits issued and an accurate record of each inspection for each permit issued, including any applicable notes. The issuance of Certificates of Occupancy will be provided to the property owner in a timely manner consistent with Chapter 14-59 (a). 5. The Contractor will accurately and on a monthly basis file all building permit related records with the appropriate state agency. 6. The Contractor may revoke permits issued, or issue a stop work order, as provided for in the Village’s Municipal Code, Chapter 14. 7. The Contractor will provide the Village Administrator with a monthly report summarizing the issuance of building permits. SERVICES TO BE PROVIDED BY THE CONTRACTOR. The Contractor shall provide security services and protection to the personnel and premises of the UNDP located at, as follows: (a) A security force of [eight (8) guards] shall patrol the and . (b) The guards shall be properly trained and licensed, in accordance with local law, to perform security services, including but not limited to, the carrying of firearms. (c) The guards shall be uniformed and wear appropriate identification badges. The names of the guards shall be given to UNDP for verification purposes well in advance of assignment to perform services under this Contract. (d) The guards shall have the following shifts: [four (4) guards at from 0600 hrs. to 1800 hrs. and four (4) guards at from 1800 hrs. to 0600 hrs.] (e) For the performance of the security services herein, the guards may be given keys to the [offices/main and rear entrance gates at UNDP Offices/ Warehouse /Residences]. In such case, the Contractor shall issue a written receipt to the guards indicating that the keys have been handed over. (f) In the event that patrol cars or other vehicles are required for the performance of security services under this Contract, the Contractor shall provide and maintain such vehicles. At the request of the Contractor, UNDP will provide radio communication equipment for each vehicle. UNDP agrees to maintain and repair the equipment as necessary. (g) The Contractor shall communicate with its security guards on duty during the day shift by and during the night shift by . (h) In the event of an emergency, the Contractor shall contact UNDP.

The Contractor shall supervise its personnel, agents and servants to ensure the performance of the Services and shall be fully responsible for all work and services performed by its employees, agents and servants.
SERVICES TO BE PROVIDED BY THE CONTRACTOR. The Contractor’s Project Manager will attend meetings, tours, and/or inspections of sites at the request of the Department. A. The Contractor shall provide all supervision, labor, materials, tools and equipment to fulfill the requirements of the Scope of Work. B. The Contractor shall implement the approved recommendations within seven (7) days of written approval from the Department’s Project Manager. The activities required to ensure the mitigation and associated Stormwater treatment areas are in compliance with the environmental permits may include, but not limited to the following: 1. Proper removal and disposal of nuisance/exotic species within designated mitigation sites, roadway ditches, or stormwater treatment areas/ponds. 2. Removal and proper disposal of dead or decaying plants. 3. Herbiciding of nuisance/exotic species within a mitigation site, stormwater treatment area, or roadway ditches. The Contractor shall be selective in herbicide application procedures (i.e. spot spraying, wicking) to assure survivability of Florida Department of Environmental Protection (FDEP) or Water Management District approved, planted or recruited species. Herbiciding will be performed by a properly licensed applicator educated in the identification of nuisance and exotic species and native wetland vegetation that may occur in the site. The Contractor will be responsible for damages to non-targeted species. Documentation of the licensing will be required. Herbicides cannot be labeled “restrictive use” and must meet all permit requirements. 4. Replacement planting within a mitigation site or stormwater treatment area. 5. Removal and proper disposal of trash or debris from a mitigation site, stormwater treatment area, or roadway ditches. 6. Maintenance of water conveyances. 7. Mowing, mulching, sodding, etc. to enhance plant growth in designated mitigation sites, stormwater treatment areas, or roadway ditches. 8. Any manual (non-mechanical) re-grading necessary. 9. Any other activity necessary to promote the survivability of a mitigation area or the operational aspect of a stormwater treatment area. 10. The Contractor will be required to perform maintenance activities including the trimming of mangroves, in accordance with FDEP requirements. SERVICES TO BE PROVIDED BY THE CONTRACTOR. The Contractor shall provide software licensing and support services supported by the Project Services as detailed in Exhibit D, and the collective documentation that comprises Exhibits D-1 through D-3. All work related to the services support is subject to prior City approval. Failure to receive approval may result in withholding compensation pursuant to §301. Contractor shall capture dispatch data from LAFD’s 9-1-1 dispatch data repository via a secure virtual private network (VPN) to VPN or Secure Sockets Layer (SSL) encrypted connection. The data captured by the Contractor will be stored in a secured database inside its firewall. The data will be accessible to authorized LAFD and County Health Department employees through a SSL connection similar to on-line banking transactions. The data will include: date and time of call, problem/nature of call, call latitude/longitude, zip code, call disposition, incident city, and incident type and other fields as agreed to by both parties. Patient information, such as names, shall not be included. LAFD and County Health Department shall define forty (40) triggers or filters that will limit and focus the types of incidents tracked, for example, flu-like symptoms. The data will be displayed on maps and charts showing geographic distributions of occurrences within the City’s geographic boundaries. Only authorized LAFD and County Health Department employees, and others authorized by the City, will have access to the charts and maps.
