Service Report. Where, in relation to the delivery of any of the Services, a Funding Schedule specifies that the Services have a measure of a service report, the delivery of those Services must be in accordance with the attached service report for the specified Service Outlet and output.

Service Report. Dedicated Service service reports shall be delivered via email, post, or other channels as specified by Customer. The Service reports will depend mainly upon the request of a customer.
Service Report. The service report will include, but not be limited to, the following information for each request for service, as applicable: • Date and time notified • Date and time of arrival • Location of Service • Client Agency contact person and telephone number • Name of technician performing the service • Description of Service Performed • Description of malfunction reported, if applicable • Diagnosis of failure and work performed, if applicable • Manufacturer and manufacturer’s part number for Fire Protection System equipment replaced, if applicable • Charges for the Service, if applicable • Departure Time The service received will not be considered complete nor will the forty-five (45) day payment period begin until all of the above is provided to the Client Agency. If requested, an additional copy of the report will be submitted to the Client Agency.

If the Contractor fails to sign in or out or leave a signed work ticket at the Client Agency’s location, whether intentional or unintentional, the Client Agency shall automatically conclude that the Service was not performed, and payment will be withheld for any such Services. Each report will be signed (printed and signed legibly) by the contact person/designee at the Client Agency’s location and by the Contractor’s technician.

Service Report. When the Supplier has completed a Delivery Item containing Service s, the Supplier shall send to the Buyer a work and services report, containing the following information: - Date - Description of Service(s) performed - Date(s) of performance - Location - Item reference (Item - serial/part number/NSN) - Problems encountered - Price where not fixed (work hours specified per staff category, spare parts and ma- terials used, etc.) - Attachments (number of attachments, description of attachments and total number of pages attached) The Buyer shall be entitled, but not obliged, to inspect the Supplier's work prior to signa- ture of the report. If the Buyer's inspection should show that the Supplier has not pe r- formed his duties as required, the Supplier shall be obliged to remedy the Defects. The Buyer's signing of the report shall not prejudice the Buyer's right to claim any reme- dy for breach.

One report shall be provided to each Camp representative detailing the results for their respective Camp and a compilation of all reports shall be provided to the Mechanical Trades Maintenance Supervisor II upon completion of testing. ▪ Training: This informal training course in industrial water treatment shall be offered as-needed for operations and maintenance personnel responsible for water-handling equipment. The training sessions shall not less than one (1) hour and offered on site at each Camp for up to 10 personnel at a time. Training materials shall be provided by the Government. The purpose of this course is to provide insight into the basic principles and fundamentals of water treatment for those who have not had the advantage of chemistry training or for those personnel that management determines require refresher training. o The training shall consist of the following topics: ❖ Purpose of water treatment ❖ Water chemistry ❖ Water impurities and the problems they cause ❖ Treatment chemicals and their function ❖ Water analysis ❖ Steam boiler water treatments, closed loop heating and cooling systems and their treatment ❖ Chemical handling and safetyChemical feed and control equipmentCorrective actions
