Security of Electronic Protected Health Information. Business Associate agrees to: (1) implement administrative, physical, and technical safeguards that reasonably and appropriately protect the confidentiality, integrity, and availability of the Electronic Protected Health Information that it creates, receives, maintains or transmits on behalf of Metro; (2) ensure that any agent, including a subcontractor, to whom it provides such information agrees to implement reasonable and appropriate safeguards to protect it; and
Security of Electronic Protected Health Information. (a) Business Associate has implemented policies and procedures to ensure that its receipt, maintenance, or transmission of “electronic protected health information” (as defined in 45 C.F.R. §160.103) (“ePHI”) on behalf of SBBC complies with the applicable administrative, physical, and technical safeguards required for protecting the confidentiality and integrity of ePHI in 45 C.F.R. Part 160 and 164 subpart C.
Security of Electronic Protected Health Information. Business Associate shall implement administrative, physical and technical safeguards that reasonably and appropriately protect the confidentiality, integrity, and availability of Electronic Protected Health Information. Business Associate shall report in writing to Company’s Corporate Compliance Office any successful Security Incident as soon as practicable but no later than five (5) days after Business Associate becomes aware of such Security Incident and shall submit follow-up documentation pursuant to the direction of Company’s Corporate Compliance Office. Upon Company’s request and pursuant to Company’s direction, Business Associate shall report in writing any attempted but unsuccessful Security Incident of which Business Associate becomes aware. Business Associate shall comply with this section 2(c)(ii) upon the later of (1) April 20, 2005; or (2) the Effective Date.
Security of Electronic Protected Health Information. To the extent that Broker receives, uses, creates, maintains and/or discloses any Electronic Health Information (“E-PHI”) in the course of providing services for or on behalf of Company, Broker additionally agrees: (i)to implement and maintain appropriate safeguards to prevent the Use or Disclosure of PHI in any manner other than as permitted by this Agreement, and to implement administrative, physical and technical safeguards that reasonably and appropriately protect the confidentiality, integrity and availability of any Electronic PHI that it creates, accesses, receives, maintains or transmits on behalf of Company; and (ii) to notify the Company if the Broker becomes aware of a Security Incident involving the Company’s E-PHI, and Broker shall provide in any notice under item (ii) above the remedial or other actions undertaken to correct the Security Incident.

Part 164, Subpart C.

Security of Electronic Protected Health Information. To the extent that Xxxxxx receives, uses, creates, maintains and/or discloses any Electronic Health Information (“E-PHI”) in the course of providing services for or on behalf of Company, Xxxxxx additionally agrees: (i) to implement administrative, physical and technical Safeguards to protect the confidentiality, integrity, and availability of the E-PHI that it creates, receives, maintains, or transmits on behalf of the Company, as required by the Security Standards; (ii) to notify the Company if the Broker becomes aware of a security incident involving the Company’s E-PHI; and (iii) to ensure that any agent, including a subcontractor, to whom it provides such E-PHI agrees to implement reasonable and appropriate safeguards to protect the Company’s E-PHI.
Security of Electronic Protected Health Information. To the extent that BUSINESS ASSOCIATE receives, uses, creates, maintains and/or discloses any Electronic Protected Health Information (“E-PHI”) in the course of providing services for or on behalf of COVERED ENTITY, BUSINESS ASSOCIATE additionally agrees: (i) to implement administrative, physical and technical Safeguards to protect the confidentiality, integrity, and availability of the E-PHI that it creates, receives, maintains, or transmits on behalf of the COVERED ENTITY, as required by the Security Standards; (ii) to notify the COVERED ENTITY if the BUSINESS ASSOCIATE becomes aware of a security incident involving the COVERED ENTITY’S E-PHI; and (iii) to ensure that any Subcontractor, to whom it provides such E-PHI agrees to implement reasonable and appropriate safeguards to protect the COVERED ENTITY’S E-PHI.
