Secondary Cardholder. You may be allowed to issue additional Cards linked to your Account to a secondary cardholder. We will treat the transactions initiated by the secondary cardholder as authorized by you and you will be liable for all transactions, fees and other liabilities related to such secondary cardholder.

You must notify us to revoke permission for any person you previously authorized to use the Card. You remain liable for any and all usage of any Additional Card you authorize. There is no fee for ordering an Additional Card for a Secondary Cardholder. A fee may be assessed for expedited delivery of an Additional Card; for more information about the delivery options and applicable fees, see the section labeled “Fee Schedule.” Your Representations and Warranties By activating the Card or by retaining, using or authorizing the use of the Card, you represent and warrant to us that: (i) you are at least 18 years of age (or older if you reside in a state where the majority age is older); (ii) you are a U.S. citizen or legal alien residing in the fifty (50) states of the United States (“U.S.”) or the District of Columbia; (iii) you have provided us with a verifiable U.S. street address (not a P.O. Box); (iv) the personal information that you provide to us in connection with the Card is true, correct and complete; (v) you received a copy of this Agreement and agree to be bound by and to comply with its terms; and (vi) you accept the Card. Address or Name Changes You are responsible for notifying us of any change in your physical address, mailing address, e-mail address, telephone number, including any number for which you have opted-in to receive text messages (“text message address”), or your name, no later than two (2) weeks after said change. Any notice of change of address, name or other contact information required by this Agreement, may be provided to us via e-mail at xxxxxxx@XxxXxx.xxx or by telephone at (000) 000-0000. Requests for address or name changes may be subject to additional verification requirements. We will attempt to communicate with you only by use of the most recent physical mailing or residential address (if different) or e-mail address or text message address we have on file for you. You agree that any notice or communication sent to you at the address noted in our records shall be effective unless we have received an address change notice from you.

You remain liable for any and all use of any additional Card(s) you authorize. PERSONAL IDENTIFICATION NUMBER You will receive a PIN once you have registered your Card with your personal information. A PIN can be used to obtain cash (see the section labeled“Cash Access”) or to make purchases at any Point-of-Sale (“POS”) device that bears the Visa, Plus®, or PULSE® Acceptance Xxxx. You should not write or keep your PIN with your Card. Never share your PIN with anyone including a Secondary Cardholder. When entering your PIN, be sure it cannot be observed by others and do not enter your PIN into any terminal that appears to be modified or suspicious. If you believe that anyone has gained unauthorized access to your PIN, you should advise us immediately by following the procedures described in the section labeled“Your Liability for Unauthorized Transfers.” To register your Card, see the section labeled“Opening a Card Account.” CASH ACCESS With your PIN, you may use your Card to obtain cash from any ATM or any POS device, as permissible by a Merchant, bearing the Visa, Plus, or PULSE Acceptance Xxxx. ATM transactions are treated as cash withdrawal transactions. Any cash withdrawn from an ATM terminal, POS device or through a participating bank or Netspend Reload Network location (an“Over-the-Counter Cash Withdrawal”) will be subject to the limitations set forth in the section below labeled“Using Your Card and Virtual Account/ Limitations”). ATM withdrawals may also be subject to varying daily limits at the ATM owner’s discretion. A fee may be associated with the use of your Card to obtain cash. For information about the fee, see the section labeled“Fee Schedule.” Note: Miles are not earned on PIN Purchase Transactions or cash withdrawal transactions. Secondary Cardholder. You may be allowed to authorize additional Cards to be linked to your Account and provided to a secondary cardholder. We will treat the transactions initiated by the secondary cardholder as authorized by you and you will be liable for all transactions, fees and other liabilities related to such secondary cardholder. Virtual Card. You may have the option of accessing your Account with, in addition to your physical card, a virtual card represented by a 16-digit account number (“Virtual Card”). The Virtual Card may either be single- use, meaning the 16-digit account number will expire after a single transaction, or recurring-use, meaning you may use your 16-digit Account number for multiple transactions. You may not use your Virtual Card to obtain cash anywhere. PIN. You will be provided with a personal identification number (“PIN”), which must be used in all automated teller machine (“ATM”) transactions and certain PIN debit transactions when requested at the point of sale. You should not disclose your PIN to anyone. If the security or confidentiality of the PIN is compromised, please notify Platform at Platform’s website and/or mobile application in order to block your Card. Alternatively, you may notify Platform at the customer service contact information set forth in Section M below.
Secondary Cardholder. Secondary cards for additional cardholders are not permitted. Should you give your Card and/or confidential PIN information to another person, you hereby acknowledge that you have authorized such person(s) to use your Card and access all of your Funds, without limit. You will be liable for all their use of the Card. Notify DCN AT ONCE if you believe your Card has been lost or stolen, or that someone has learned your confidential PIN and/or Card Number. The best way of minimizing your possible losses is to use our 24 hour automated telephone service at (000) 00-XXXXX (866-783- 3248), or log into your Online Cardholder Account at xxx.xxxxxxxxxxxxxxx.xxx to report your card lost or stolen. If you need to speak to a representative, you may call customer service during our normal business hours at (000) 000-0000 (see section below, 32. Business Days and Hours). PERSONAL IDENTIFICATION NUMBER (“PIN”) NOTICE THIS AGREEMENT CONTAINS A PERSONAL IDENTIFICATION NUMBER (“PIN”) CLAUSE. The minimum amount of the initial value load is $1.00. The minimum amount of each value reload is $1.00. The maximum amount of the initial value load is $99,999.99. The maximum amount of each value reload is $99,999.99. You will have access to your funds no earlier than the start of your Program Sponsor’s Plan start date or upon eligibility as described in the Plan documents. Using Your Card/Features The maximum amount that can be spent on your Card per day is the available balance on your Card Account. The maximum value of your Card is restricted to $999,999.99. The Card allows you to make purchases based on the available balance associated with your Plan. You may use your Card to pay for eligible healthcare expenses (“Eligible Expenses”) at merchants that sell products and services that qualify as Eligible Expenses and that accept Visa Debit cards (“Eligible Merchants”) as long as you do not exceed the value available on your Card Account. Note: The Card cannot be used wherever Visa Debit cards are accepted. If you do not wish to use the card, you may be able to obtain reimbursement of Eligible Expenses by means of a paper claims process. You have received the Card as a means of convenient and secure access to a fixed dollar amount representing the available balance in your Plan. You agree that (a) you are a U.S. citizen or legal alien resident in one of the 50 U.S. states or the District of Columbia and have a verifiable U.S. mailing address; (b) you are at least 18 years of age; (c) you will not use or attempt to use your Card to make purchases other than for Eligible ...
