Screen Failures. Estimated [NUMBER] screen failures per site will be provided for, if necessary, at the rate of the screening visit and all performed procedures. The cost of the screening failure will be calculated depending on the procedures performed until the Trial Subject failed the screening. xxxxxxx Any screen failure exceeding this limit will require prior written Sponsor/INC Research approval. To be eligible for reimbursement of Screen failure, the Institution must: (I) allow trial monitor to conduct source document verification for screening, (ii) employ reasonable screening procedures and processes that only appropriate subjects are entered into the screening procedures, and (iii) have proper documentation available for verification, if requested. Subjects recruited due to violation of the inclusion/exclusion criteria will not be eligible for screen failure payments. A-6. Každý neúspěšný screening převyšující tento limit bude vyžadovat předchozí písemné schválení zadavatele/INC Research. Aby bylo zdravotnické zařízení způsobilé pro úhradu neúspěšného screeningu, musí: (i) umožnit monitorovi klinického hodnocení provést verifikaci zdrojových dokumentů, (ii) používat odůvodněné screeningové postupy a procesy, aby screeningové úkony podstoupily pouze vhodné subjekty hodnocení a (iii) mít k dispozici správnou dokumentaci pro účely verifikace, pokud bude požadována. Subjekty, při jejichž získání byla porušena kritéria pro zařazení/vyloučení, nebudou způsobilé pro platbu za neúspěšný screening.

Osoba, ktorá nesplní kritériá spôsobilosti, je Xxxxxxxx osobou, ktorá udelila súhlas, ale nesplnila podmienky preverovacej návštevy, a preto nie je oprávnená na zápis do Skúšania. Za Osoby, ktoré nesplnili kritéria pri preverení, budú preplácané náklady (ak také osoby budú) tak, ako je to stanovené v Prílohe D, a to na základe prác, ktoré boli dokončené podľa Protokolu.
