Schedule of Insurance. Certificate means a certificate issued by Us, and, if more than one, then the latest in time. The Schedule of Insurance Certificate contains details of the Policyholder, Insured Persons and the Benefits applicable under the Policy.

Schedule of Insurance. The benefits for which a protected person is covered will be those described in the following Schedule of Insurance. Monthly Income Benefit: 70% of monthly earnings in effect immediately prior to the date of disability. The maximum monthly income benefit is $6,000.00. Evidence of Insurability satisfactory to the Company is required for amounts in excess of $4,700.00
