Sanctioned Individuals. Upon any renewal or extension of this Contract and at any time upon a written request by EOHHS, the Contractor shall furnish full and complete information to EOHHS as required by 42 CFR 1002.3, regarding sanctioned individuals as described under 42 CFR 1001.1001(a)(1).

Sanctioned Individuals. Omnibus Account Holder shall not submit, and shall not permit the submission of, any funds of any Omnibus Account Holder client or any of its clients’ order to IBAU until it has determined that the client’s name does not appear on sanction list issued by a FATF country, including but not limited to the SDN List issued by the US Department of Treasury's Office of Foreign Assets Control ("OFAC") and the Consolidated Sanction List issued by the United Nations Securities Council.
Sanctioned Individuals. FRHS has policies and procedures in place to ensure that it does not employ or contract with any individual or entity that is excluded from participating in any federally funded healthcare program.
