Safety Reporting a. Licensee is responsible for all single and periodic reporting to all applicable regulatory authorities for the Products manufactured by or on behalf of Licensee under the Agreement.
Safety Reporting. Each employee shall be responsible for inspecting the equipment, vehicle and job site to which the employee is assigned, and to report any deficiencies discovered to appropriate management personnel. An employee who finds the equipment, vehicle or job site to which the employee is assigned to be in an unsafe or inoperable condition, posing threat of immediate and serious physical injury, shall report such finding to the immediate supervisor. Upon being so notified, the supervisor or his designee shall inspect the condition and determine if it is safe and operable. The supervisor shall notify the employee of the decision in writing. If the employee still feels that the condition was unsafe, then the employee may report the condition to the Department of Environmental Health and Safety. The Department of Environmental Health and Safety will investigate the condition and make its recommendations in writing. If the condition is not corrected, the employee may file a grievance. Abuse of this section may be cause for disciplinary action.
Safety Reporting. All employees shall report promptly unsafe working conditions to their supervisors. Any employee involved in an accident shall report the accident and any injuries sustained according to Employer procedures. The employee and the supervisor shall immediately make out an accident report in a form provided by the Employer, including notation of all witnesses to the accident.
Safety Reporting. The Shasta County Injury Illness Prevention Program Policy (IIPP) (available on the Support Services / Risk Management Intranet web site for reference) states that County employees are responsible for ensuring their own safety and the safety of others by: Bringing to the supervisor’s attention any activity, behavior, or unsafe condition which would cause injury or illness to others or damage property. The Union and the County agree that as an alternative to the supervisor, the employee may fulfill his/her responsibility by immediately bringing such matters to the attention of the Department Safety Coordinator (described as the Department Safety Representative in the IIPP).
Safety Reporting. The following language is hereby added to the Agreement as Section 6.6:
Safety Reporting. Contractor shall submit monthly safety reports to KO and COR. Contractor will submit the report no later than the 8th of each month.
Safety Reporting. Institution and Investigator will report all serious adverse events, Special Situation Reports (defined below), and other safety concerns as specified in the Protocol and in accordance with applicable laws, rules and regulations, including, but not limited to ICH-E2A and the European Union Directive 2001/20/EC. For the avoidance of doubt, each of the Investigator and the Institution will provide all reasonable assistance to Gilead to allow Gilead to comply with all applicable requirements. “Special Situation Reports” means: a) pregnancy reports; b) reports of medication error, abuse, misuse, or overdose; c) lack of effect reports, d) reports of adverse reactions in infants following exposure from breastfeeding; e) reports of adverse reactions associated with product complaints; and f) reports arising from occupational exposure. 1.9
Safety Reporting. Transmission Owner shall notify Transco of all safety “near misses”, OSHA recordables and OSHA reportables as set forth in §4.02(a). Transmission Owner shall provide Transco a copy of all written reports made to OSHA.
Safety Reporting. Each Party will advise the other Party in writing of any adverse event related to the Vaccine Components or Product within [***] after becoming aware of such event, and provide any and all information, document, and materials that are related to such adverse event. Further details of the Parties’ obligations in regard to safety reporting will be set forth in the Pharmacovigilance Agreements.
Safety Reporting. All employees shall promptly report unsafe working conditions, activities or equipment to their immediate supervisors. If the immediate supervisor does not xxxxx the problem within a reasonable time, an employee may report the unsafe condition to the chief of the section. An employee shall not be disciplined for reporting unsafe conditions. The appropriate supervisor shall attempt to xxxxx the problem or will report to the employee or his representative in five (5) days or less reasons why the problem cannot be abated in an expeditious manner.
