Risks and Inconveniences. Like other orthodontic treatments, the use of AcceleDent Aura may involve some of the risks outlined below: • Failure to use the device as prescribed (typically once per day for 20 minutes) and directed by your orthodontist may not reduce the treatment time to achieve the desired results. • Overuse of the device (using for more than 20 minutes per day) has not been evaluated and therefore may not be safe. • Although not reported in the clinical studies, AcceleDent Aura could theoretically cause slight discomfort of the teeth or a headache. • Although not reported in the clinical studies, AcceleDent Aura could theoretically damage your orthodontics, especially if used incorrectly. • You will not be able to talk, sleep, or eat for the 20 minutes that the AcceleDent Aura is in use. • As with any orthodontic treatment, a tooth that has been previously traumatized, or significantly restored, may be aggravated. In rare instances the useful life of the tooth may be reduced, the tooth may require additional dental treatment such as endodontic and/or additional restorative work and the tooth may be lost. • Although not reported in the clinical studies, existing dental restorations (e.g. crowns) may become dislodged and require recementation or in some instances, replacement. • Although study data indicate AcceleDent Aura does not affect the length of the roots of teeth, the length of the roots of the teeth may be shortened during standard orthodontic treatment and may become a threat to the useful life of teeth. • General medical conditions and use of medications can affect orthodontic treatment. • Care should be taken with the AcceleDent Aura - product breakage can occur if not properly handled. • Although not reported in the clinical studies, allergic reactions may occur with any product – talk to your orthodontist if you have allergy concerns. Informed Consent I have been given adequate time to read and have read the preceding information describing orthodontic treatment and use of the AcceleDent Aura accessory device. I understand the benefits, risks, and inconveniences associated with treatment. I have been sufficiently informed and have had the opportunity to ask questions and discuss concerns about orthodontic treatment with the AcceleDent Aura System with my doctor from whom I intend to receive treatment.   • REIMBURSEMENT FOR PROPERTY DAMAGE In the event that an employee, required or authorized by his/her Agency/Department Head to use a private automobile on County business, while so using the automobile, should incur property damage to the employee's automobile through no negligence of the employee, and the employee is unable to recover the cost of such property damage from either his/her own insurance company or from any other driver, or other source, such costs shall be paid to such employee of the County, in a sum not exceeding $500, provided that any claims the employee may have against his/her insurance company or any third party have been litigated or settled, and provided further, that the employee is not found guilty of a violation of the California Vehicle Code or Penal Code in connection with the accident causing such damage. Employees shall submit proof of loss, damage or theft (i.e., appropriate police report and/or estimated statement of loss) to the Agency/Department Head within 30 days of such loss, damage or theft. Property damage or loss incurred to the private automobile while located on the street or at the parking facility serving the employee’s normal place of work shall not be compensated under this section, but property damage or loss incurred to the private automobile while located on the street or at the parking facility serving the employee’s County business destination shall be compensated as provided above.

  • Other Methods of Procurement of Consultants’ Services The following table specifies methods of procurement, other than Quality and Cost-based Selection, which may be used for consultants’ services.   • DAMAGE TO FACILITIES The Student will promptly report damages and request necessary repairs, in accordance with established and published procedures. The Student is responsible for damages caused by the Student. Damages to shared or common areas not attributable to a responsible person are the joint responsibility of all persons sharing the space or area. UCF DHRL reserves the right to charge the Student for damages caused by the Student. Charges may include, but are not limited to, extraordinary pest control charges, repair of walls, furniture or fixtures, and damages to common areas. UCF DHRL also reserves the right to charge the Student for a portion of damages caused to spaces shared by the Student when the person responsible for damages to shared spaces cannot be identified.

  • Lost Keys, Lock Outs The Resident will be responsible for the cost of replacing lost keys (e.g., room key, mailbox key, etc.) at a cost determined by the Manager, to a maximum of $20.00 per key. If the Resident is locked out of the Resident’s Room, the Resident will pay a fee for letting the Resident into the Room, to a maximum of $5.00, or will be provided with a temporary key to be returned immediately after use. Failure to return a temporary card in the time allotted will result in a replacement fee to a maximum of $20.00.
