REVIEW OF PLAN OF CARE. (a) Hospice staff shall review the integrated plan of care at least every 2 weeks after the initiation of services or at more frequent intervals specified in the plan, and more frequently in response to a significant change in the individ- ual’s condition. If the core team determines that the written goals and interventions are not effective in meeting the needs of the patient or the patient’s family, or both, or that their needs have changed, the plan of care shall be revised to provide for updated goals and intervention strategies to meet those needs. Any revi- sion shall include outcomes expected and dates for evaluation as provided under sub. (3) (b) 5. All members of the hospice core team shall participate in the plan of care review. The plan of care shall be updated as necessary.

  • SUPERVISION AND CONSTRUCTION PROCEDURES 4.3.1 The Contractor shall supervise and direct the Work, using his / her best skill and attention. The Contractor shall be solely responsible for all construction means, methods, techniques, sequences and procedures and for coordinating all portions of the Work under the Contract.   • Review Procedures The Parties agree to jointly conduct a review, sampling transactions of the incidents managed under this Agreement. Findings that are inconsistent with the normal or accepted way of doing business will be reconciled on a case by case basis. Any decision to further examine records will be considered on a case by case basis and appropriate follow up action agreed upon by all agencies involved. Payment for Protection Services (use if appropriate) Geographic, Statewide or Sub-Geographic (local) operating plans and procurement documents or agreement will establish billing procedures for Fee Basis Protection Services.
