RETIREE MEDICAL FOR EMPLOYEES. HIRED ON OR AFTER JANUARY 1, 2015. Employees hired on or after January 1, 2015, who have a minimum of five (5) years of service with the ACFD, will receive a retiree medical benefit based on the years of service. Additional years with a public sector agency that contracts with PERS for retirement benefits shall apply.

RETIREE MEDICAL FOR EMPLOYEES. Hired After June 26, 2006 and Before November 1, 2010 (Tier 2): Employees hired on or after June 26, 2006 (the date of implementation of the 3.0% @50 Retirement Benefit) and before November 1, 2010, will receive retiree medical contributions based on years of service with the Police Department. The retiree medical contribution for employees who have a service retirement will be as follows: Years of Service Monthly Contribution 0-end of 9th year of service Minimum monthly amount as governed by the CalPERS Health System. 10 years to the end of the 14th year of service 50% of the lowest medical premium provided through CalPERS approved medical providers for employee +1 dependent. 15 years to the end of the 19th year of service 75% of the lowest medical premium provided through CalPERS approved medical providers for employee +1 dependent. 20 years of service or more 100% of the lowest medical premium provided through CalPERS approved medical providers for employee +1 dependent.
RETIREE MEDICAL FOR EMPLOYEES. Hired Prior to June 26, 2006 Who Retired Prior to January 1, 2017 (Tier 1): Employees hired prior to June 26, 2006 who retire prior to January 1, 2017 with a minimum of five (5) years of service with the City will receive a retiree medical benefit equivalent to the amount necessary for actual enrollment in single, two-party, or family coverage, up to a maximum dollar amount of the Kaiser family premium rate.
RETIREE MEDICAL FOR EMPLOYEES. HIRED PRIOR TO APRIL 1, 2009 Employees hired prior to April 1, 2009, with a minimum of five (5) years of service with the Department, will receive a stipend amount, less the Minimum Employer Contribution, equal to the costs of the premium for the medical plan selected, up to the amount necessary for actual enrollment in Kaiser Single, Kaiser Two-Party, or Kaiser Family. For eligible retirees who are 65 years of age or older and enrolled in Medicare, the Departments contribution will be a stipend amount, less the Minimum Employer Contribution, equal to the costs of the premium for the medical plan selected, up to equivalent to the premium for Kaiser Single, Kaiser Two-Party, or Kaiser Family Medicare medical coverage as applicable.

RETIREE MEDICAL FOR EMPLOYEES. Hired On or After March 31, 2008 and Before August 15, 2011 (Tier 2): Employees hired on or after the effective date of the 2.5% at 55 retirement formula (March 31, 2008) will receive a retiree medical benefit based on years of service with the City as follows: • 0-9 years of service = minimum monthly amount as governed by the CalPERS Health System. • 10 years of service = an amount equal to 100% of employee only lowest cost premium • 15 years of service = an amount equal to 75% of employee + 1 lowest cost premium • 20 years of service = an amount equal to 100% of employee + 1 lowest cost premium

For eligible retirees who are under age 65, the City will contribute up to the Kaiser Family premium for retiree plus two or more. Xxxxxxxx retirees who are 65 years of age or older must enroll in Medicare. If their spouse/dependents are under the age of 65, the City will contribute up to the actual enrollment cost of the Medicare combination supplement plan premium for the Bay Area Region for the third highest cost plan for single retirees; the third highest cost plan for retiree plus one; or Kaiser Family for retiree plus two or more. For eligible retirees who are 65 years of age or older and enrolled in Medicare, and their spouse/dependents are also 65 years of age or older, the City contribution will be up to the actual enrollment cost of the medical premium for CalPERS Bay Area Region Medicare Supplemental Premium for all levels of coverage. o Single Coverage: third highest cost plan o 2-Party Coverage: third highest cost plan o Family Coverage: Kaiser Xxxxxxxx retiree is defined as the former City employee with the requisite years of City service enrolled in a CalPERS plan.

All contributions listed below include the PERS required Minimum Employer Contribution (MEC). • 10 years of service = the actual enrollment cost of an amount up to or equal to 100% of employee only lowest cost premium, less PERS required MEC o For eligible retirees with 10-14 years of service who are under age 65 and not yet Medicare eligible, the lowest cost premium referenced above will be tied to the lowest cost Basic CalPERS Bay Area Region premium. o For eligible retirees with 10-14 years of service who are Medicare eligible, the lowest cost premium referenced above will be tied to the CalPERS Bay Area Region Medicare Supplemental premium. • 15 years of service = the actual enrollment cost of an amount up to or equal to 75% of employee + 1 lowest cost premium, less PERS required MEC • 20 years of service = the actual enrollment cost of an amount up to or equal to 100% of employee + 1 lowest cost premium, less PERS required MEC o For eligible retirees with 15 or more years of service who are under age 65 and not yet Medicare eligible, regardless of the age of their spouse/dependents, the lowest cost premium referenced above will be tied to the lowest cost Basic CalPERS Bay Area Region premium. o For eligible retirees with 15 or more years of services who are Medicare eligible, if their spouse/dependent is under the age of 65, meaning the eligible retiree and eligible dependents are enrolled in a Medicare Combination Plan, the lowest cost plan referenced above will be tied to the CalPERS Bay Area Region Medicare Combination Supplemental plan. o For eligible retirees with 15 or more years of services who are Medicare eligible, and their spouse/dependent is also 65 years of age or older, the lowest cost plan referenced above will be tied to the CalPERS Bay Area Region Medicare Supplemental Plan. Xxxxxxxx retiree is defined as the former City employee with the requisite years of City service enrolled in a CalPERS plan.
RETIREE MEDICAL FOR EMPLOYEES. Hired Prior to June 26, 2006 Who Retire On or After January 1, 2014 (Tier 1a): Effective January 1, 2014, employees hired prior to June 26, 2006, who retire from the City with five (5) years of City service, will receive a retiree medical benefit in accordance with the following: • For eligible retirees, the City contribution will be equivalent to the Bay Area Region premiums for Blue Shield Access HMO Single, Blue Shield Two-Party, or Kaiser Family coverage as applicable. • For eligible retirees who are 65 years of age or older and enrolled in Medicare, or a Medicare Combination plan, the City contribution will be equivalent to the Medicare, or Medicare Combination, supplement plan premium for the Bay Area Region for Blue Shield Access HMO Single, Blue Shield Two-Party, or Kaiser Family coverage as applicable. If the Blue Shield Access HMO or Kaiser is no longer offered by CalPERS medical, the employee will receive the contribution equal to the third highest cost plan offered by XxxXXXX medical.
