Retention of PHI. Except upon termination of this Agreement as provided in Section 5, below, Business Associate and its Subcontractors or agents shall retain all PHI throughout the term of this Agreement, and shall continue to maintain the accounting of disclosures required under Section 3.h, above, for a period of six years.
Retention of PHI. Business Associate shall retain all PHI throughout the term of the Master Agreement and shall continue to maintain the information required under Section 2(h) of this BA Agreement for a period of six (6) years from its creation.
Retention of PHI. Notwithstanding Section 6(c) of this BAA, Business Associate and its Subcontractors shall continue to maintain the information required under Section 3(i) of this BAA for a period of six (6) years after disclosure of the PHI.
Retention of PHI. Notwithstanding Section 4(c) of this Agreement, Business Associate shall only retain PHI throughout the term of the Services Agreement as necessary to perform the Services and upon termination or expiration of this Agreement all PHI shall be returned to the Covered Entity or destroyed in accordance with section 4.c of this Agreement.
Retention of PHI. Notwithstanding section 4.a. of this Addendum, Associate and its subcontractors or agents shall retain all PHIS pursuant to state and federal law and shall continue to maintain the PHI required under Section 3.f. of this Addendum for a period of six (6) years after termination of the Agreement, or longer if required under state law.
Retention of PHI. SART shall retain all PHI throughout the term of this Agreement and shall continue to maintain the information required under Section 3(g) of this Agreement for a period of six (6) years from its creation.
Retention of PHI. Business Associate and its subcontractors or agents shall retain all PHI pursuant to state and federal law and shall continue to maintain the PHI required under this Agreement for a period of six (6) years after termination of the Agreement, or longer if required under state law.
Retention of PHI. BA shall retain all PHI throughout the term of this Agreement and shall continue to maintain such information not otherwise returned or destroyed pursuant to Section 5.4 of this Agreement for a period of six (6) years after the termination of this Agreement.
Retention of PHI. Business Associate and its agents or Subcontractors shall retain all PHI and documentation containing Disclosures of PHI throughout the term of the Service Agreement and for a period of six (6) years after termination of the Service Agreement.
Retention of PHI. Plan Sponsor and/or any Authorized Employee will, if feasible, return or destroy all such PHI that it maintains in any form and retain no copies of such PHI when no longer needed for the purpose for which disclosure was made. If such return or destruction is not feasible, Plan Sponsor and/or any Authorized Employee will limit further uses and disclosures to those purposes that make the return or destruction of the information infeasible.
