Resume Self-Certification Form. Innova, Inc. will make sure each candidate to complete read the Resume Self-Certification form and explain the consequences of them caught lying or providing false information in their resume. We will make sure the candidate signs the Self-Certification Form, before submitting them to customer for further review. We will keep an original self-certification copy in our records.
