Identification of health conditions that require prompt medical attention such as acute illness, chronic disease(s) requiring immediate medical management and/or treatment (e.g. asthma, diabetes, seizure disorder), signs of infection or communicable disease, nutritional problems, pregnancy, and significant developmental or mental health conditions.

At a minimum, the screening must include questions that enable the HMO to identify the following: • The member’s chronic physical, mental and behavioral health illness(es) (e.g. respiratory disease, cardiac disease, stroke, diabetes/pre- diabetes, renal disease, back pain and musculoskeletal disorders, cancer, overweight/obesity, all mental health and substance abuse disorders). • The member’s perception of their strengths, their general well-being (including chronic conditions and access to prescription medications). • If the member has a usual source of care. • Any indirect supports the member may have (family and social supports). • Any relationships the member may have with community resources. • Any immediate and/or long-term concerns a member may have about their overall well-being (e.g. social determinants of health). • If the member needs assistance to conduct activities of daily living (including but not limited to bathing, dressing and eating) as well as instrumental activities of daily living (including but not limited to medication management, money management and transportation).

The Tiered Trigger Level Framework (the “Trigger Framework”) shall be comprised of the following types of Trigger Levels: Instream Surface Water Trigger Levels, Trigger Levels for Direct Discharges, and Trigger Levels for Indirect Discharges.
