Common use of Request for Exclusion Clause in Contracts

Request for Exclusion. “Request for Exclusion” refers to a timely, written, opt- 25 out request signed by a Class Member and submitted to the Settlement Administrator with the 26 following information: (1) the Class Member’s full name and current mailing address, (2) the last four 27 digits of the Class Member’s social security number, and (3) an express statement that the Class 28 Member wishes to be excluded from the terms of the Agreement.
