REPORTING DAMAGES. If any damage (including death, personal injury or property damage) occurs in connection with the performance of this Agreement, Developer shall immediately notify the City Risk Manager’s office by telephone at ▇▇▇-▇▇▇-▇▇▇▇, and Developer shall promptly submit to the City’s Risk Manager and the City Manager or designee, a written report (in a form acceptable to the City) with the following information: (a) a detailed description of the damage (including the name and address of the injured or deceased person(s), and a description of the damaged property), (b) name and address of witnesses, and (c) name and address of any potential insurance companies.

REPORTING DAMAGES. If any damage (including death, personal injury or property damage) occurs in connection with the performance of this Agreement, Artist shall immediately notify the City Risk Manager’s office by telephone at ▇▇▇- ▇▇▇-▇▇▇▇, and Artist shall promptly submit to the City Risk Manager and the City’s Authorized Representative, a written report (in a form acceptable to the City) with the following information: (a) name and address of the injured or deceased person(s), (b) name and address of Artist subcontractor, if any, (c) name and address of Artist’s liability insurance carrier, and (d) a detailed description of the damage and whether any City property was involved.

REPORTING DAMAGES. If any damage (including death, personal injury or property damage) occurs in connection with the performance of this Agreement, Operator shall immediately notify the City Risk Manager’s office by telephone at ▇▇▇-▇▇▇-▇▇▇▇, and Operator shall promptly submit to the City’s Risk Manager and the City’s Authorized Representative, a written report (in a form acceptable to the City) with the following information: (a) name and address of the injured or deceased person(s), (b) name and address of witnesses, (c) name and address of Operator's insurance company, and (d) a detailed description of the damage and whether any City property was involved.

REPORTING DAMAGES. If any damage (including death, personal injury or property damage) occurs in connection with the performance of this Agreement, Contractor shall immediately notify the City Risk Manager’s office by telephone at ▇▇▇-▇▇▇-▇▇▇▇, and Contractor shall promptly submit to the City’s Risk Manager and the City’s Administrator (see Paragraph 17, hereinbelow) a written report (in a form acceptable to the City) with the following information: (a) name(s) and address(es) of the injured or deceased person(s), (b) name(s) and address(es) of witnesses, (c) name and address of Contractor’s insurance company, and (d) a detailed description of the damage and whether any City property was involved.

REPORTING DAMAGES. If any damage (including but not limited to death, personal injury or property damage) occurs in connection with the performance of this Agreement, Consultant shall immediately notify the City Risk Manager’s office by telephone at ▇▇▇-▇▇▇-▇▇▇▇, and Consultant shall promptly submit to the City’s Risk Manager and the City’s Administrator (see paragraph 19, herein below) a written report (in a form acceptable to City) with the following information: (a) name(s) and address(es) of the injured or deceased person(s), (b) name(s) and address(es) of witnesses, (c) name(s) and address(es) of Consultant’s insurance company(ies), and

REPORTING DAMAGES. The tenant must report any damages to the property as soon as they occur. * If damages are not reported promptly, the tenant may be held liable for them. 4.

REPORTING DAMAGES. In the event that either party becomes aware of any actual or potential damages arising out of or in connection with this Agreement, that party shall promptly notify the other party in writing, providing detailed information regarding the nature and extent of the damages. Both parties agree to take all reasonable steps to mitigate any damages incurred or that may be incurred as a result of any breach of this Agreement or any other act or omission relating to this Agreement.
