Reimbursement Claims. Submit a Claim for Reimbursement within 35 days of the end of the calendar quarter in which expenses were incurred (submit claims no more than monthly), using the form provided by Agency as follows:

Reimbursement Claims. Settlement Class Members who paid for an inspection pursuant to the Inspection Program and the Inspection Notice Letter prior to [Preliminary Approval Date] can file a claim (a “Reimbursement Claim”) for their unreimbursed, out-of-pocket inspection fee of $80.00.

Reimbursement Claims. The SFA shall be responsible for signing reimbursement claims. This responsibility cannot be delegated to FSMC. 7 CFR 210.16(a)(5).
Reimbursement Claims. Neither the Borrowers nor any of the Subsidiaries has received any notice or correspondence, nor are they aware of any basis for receiving the same, that any Medicare, Medicaid or any other payor programs have any claims against it that could result in a net offset against past or future reimbursement of the Borrowers or the Subsidiaries not otherwise reflected in the Financial Statements. To the knowledge of the Borrowers, neither the Borrowers nor any of the Subsidiaries nor any of their respective employees (while acting in the capacity of an employee of the Borrowers or any of the Subsidiaries) has committed a violation of the Medicare and Medicaid fraud and abuse provisions of The Social Security Act or the Civil Monetary Penalties Law of the Social Security Act.
