Purpose of the Disclosure. Agreement This Disclosure Agreement is being executed and delivered by the County and the Dissemination Agent for the benefit of the Holders and Beneficial Owners of each Series of 2010 Bonds and in order to assist the Participating Underwriters in complying with S.E.C. Rule 15c2-12(b)(5).
Purpose of the Disclosure. The Contractor agrees to collect and use any data disclosed to it pursuant to this Agreement solely for the purposes of BE ENGAGED®, an initiative focused on encouraging parents, families and communities to work together to transform student outcomes. The Contractor, through the PimsPoints application and/or website, allows parents/guardians voluntarily creating accounts in the application to earn points for attending activities and meetings, workshops/trainings, volunteering and/or participating in extracurricular activities, participating in surveys, e-signing and submitting important documents, and completing learning tasks at home. The application and/or website is an interactive communication tool between schools and/or school systems and parents/guardians participating in the application.
Purpose of the Disclosure. The Contractor agrees to collect and use any data disclosed to it pursuant to this Agreement solely for the purposes of providing information to student parents to support their child’s classroom learning at home.

Purpose of the Disclosure. The Contractor agrees to collect and use any data disclosed to it pursuant to this Agreement solely for the purposes conducting administrative reviews of all school food authorities participating in the following Child Nutrition Programs (CNP): Child and Adult Care Food Program (CACFP) Summer Food Service Program (SFSP) National School Lunch Program (NSLP) and the Afterschool Snack Program (ASP) School Breakfast Program (SBP) Fresh Fruit and Vegetable Program (FFVP) Seamless Summer Option (SSO) The State will not have access to personally identifiable student data except as provided in La. R.S. 17:3914 for auditing, at the direction of the LEA Superintendent, or where parental consent is provided
Purpose of the Disclosure. The Contractor agrees to collect and use any data disclosed to it pursuant to this Agreement solely for the purposes of the providing service for diagnosing and identifying students in need of tutoring and to evaluate the effectiveness of tutoring intervention. .
Purpose of the Disclosure. Contractor agrees to collect and use any data disclosed to it pursuant to this Agreement solely for the purposes of evaluating the effect of the Louisiana Literacy Coach Program.
Purpose of the Disclosure. Contractor agrees to use any data disclosed to it by the State pursuant to this Agreement solely for the purposes of providing the services specified in the Exam Agreement, as well as troubleshooting, maintenance and product-improvement activities related thereto. Contractor shall be a processor and State the controller of any data provided pursuant to this Agreement.
Purpose of the Disclosure. The Contractor agrees to collect and use any data disclosed to it pursuant to this Agreement solely for the purposes of the providing the following: Contractor agrees to collect and use any data disclosed to it pursuant to this Agreement for the purposes of the providing the services to State and LEA related to the English Language Proficiency Test (ELPT) and the English Language Proficiency Screener.
Purpose of the Disclosure. With LEA permission, Contractor agrees to share data collected under direct agreements between Contractor and LEAs for ACT and ACT WorkKeys Assessments with the State for the purposes of state assessment and accountability process. Student data provided directly to Contractor by the student or the student’s parent/guardian is not subject to this Agreement.
