Purpose of the Data Sharing. 2.1 Framework This Data Sharing Agreement sets out the framework for the sharing of personal data between the Parties and defines the principles and procedures that the Parties shall adhere to and the responsibilities the Parties owe to one another. This agreement is required to ensure that any sharing of personal data is carried out in accordance with the GDPR and the Data Sharing and Governance Act 2019, and each Party agrees to be bound by this agreement until such time as the agreement is terminated, or the Party withdraws from the agreement. The Parties shall not process shared personal data in a way that is incompatible with the relevant purposes and this agreement. The Parties will ensure that the Data Sharing Agreement remains fit for purpose, accurate and up to date. The Parties will actively monitor and periodically review the data sharing arrangement to ensure that it continues to be compliant with data protection law, that it continues to meet its objective, that safeguards continue to match any risks posed, that records are accurate and up to date, that there is adherence to the data retention period agreed and that an appropriate level of data security is maintained. The Parties must address all recommendations made regarding this Data Sharing Agreement by the Data Governance Board.

The purpose of this Agreement is to support the information sharing intent expressed in Section XI of the general interagency agreement between these Parties as signed on August 9, 2005. Specifically, to provide DJJ “read only” access and print capabilities to the data contained in the child welfare information system(s) maintained by DCF. For the purposes of this Agreement, the child welfare information systems include the HomeSafenet (HSn) and its successor the Florida Safe Families Network (FSFN), and any succeeding systems. These are also generically known as SACWIS, and include the information collected for child protection by the Florida Abuse HotLine. Pseudonymised Data for place-based local Intelligence Services: This will include CCG, Local Authority and the Cheshire and Merseyside BI Teams supporting the Out of Hospital cell Pseudonymised data that cannot be re-identified for Research Organisations. The purposes of data for local Intelligence Services are those described in the COPI notice for COVID-19 • Understanding COVID-19 and risks to public health, trends in COVID-19 and such risks, and controlling and preventing the spread of COVID-19 and such risks; • Identifying and understanding information about patients or potential patients with or at risk of COVID-19, information about incidents of patient exposure to COVID- 19 and the management of patients with or at risk of COVID-19 including: locating, contacting, screening, flagging and monitoring such patients and collecting information about and providing services in relation to testing, diagnosis, self-isolation, fitness to work, treatment, medical and social interventions and recovery from COVID-19; • Understanding information about patient access to health services and adult social care services and the need for wider care of patients and vulnerable groups as a direct or indirect result of COVID-19 and the availability and capacity of those services or that care; • Monitoring and managing the response to COVID-19 by health and social care bodies and the Government including providing information to the public about COVID-19 and its effectiveness and information about capacity, medicines, equipment, supplies, services and the workforce within the health services and adult social care services; • Delivering services to patients, clinicians, the health services and adult social care services workforce and the public about and in connection with COVID-19, including the provision of information, fit notes and the provision of health care and adult social care services; and • Research and planning in relation to COVID-19
Purpose of the Data Sharing. Purpose of Data Sharing The purpose of the data sharing is for direct care within the context of a Shared Care Record (ShCR). The aim of implementing a shared record platform into health and social care organisations is to improve the quality, safety and coordination of care received by individuals across Cheshire, Merseyside. This is achieved through ensuring health and care professionals are able to have the right information, at the right time, regardless of where the individual is receiving care and support. We aim to ensure all care settings are able to collaborate and coordinate care, whether it is within a GP practice, hospital, community site or within the home. Throughout the lifecycle of the programme, patients will be informed of the benefits of sharing, and the programme will work to ensure we can interoperate with key systems across the geography, ensuring a wealth of clinically relevant data is available to their health and care providers. To note: This data set cannot be used for research. Data to be Shared Care Centric (supplied by Graphnet) Appendix A shows the data items that are to be shared within Care Centric. Wirral Health Information Exchange platform (supplied by Cerner) Appendix B shows the data items that are to be shared within Cerner. e-Xchange Appendix C shows the data items that are to be shared within e- Xchange.
Purpose of the Data Sharing. Scotland’s Adoption Register is a free service being hosted by St Xxxxxx’s Children’s Society and funded by the Scottish Government for the purposes of facilitating adoption in accordance with section 13A of the 2007 Act. St Xxxxxx’s Children’s Society is authorised to establish and maintain the register on behalf of the Scottish Ministers under section 13B of that Act. As specific aspects are completed by different partners, it is necessary for relevant data to be shared between those partners throughout the survey cycle. For example, SQA collect the assessment data for the SSLN which is then analysed by EAS for the report. EAS then provide analysis data on the results of the survey to ES to be used in the PLRs. Sharing the survey data between these partners allows a report on the national performance of literacy and numeracy to be published by EAS, as well as PLRs to be published by ES, based on the survey data. Once they become data controllers, ES may wish to commission analysis of the data for the PLRs. If this is the case, it will be necessary for a Confidentiality Agreement to be arranged by ES and signed by the third party. ES will have responsibility for the data whilst it is being analysed by any third party.
