This officer will conduct and participate in prevention and awareness education and interact with staff and students to promote a positive, safe, and secure educational environment.
Purpose and Mission. The WCU CMHS is a training site associated with the WCU doctoral program in clinical psychology. Clinicians are clinical psychology graduate students who are supervised by clinical staff and faculty. Your student clinician will provide you with the name of his or her supervisor. All evaluation and therapy sessions are video recorded for the purposes of student training. The student clinician, supervisor and small team of clinical graduate students may view the recordings and discuss your case as part of their training. All recordings will be deleted within 60 days. The only exception is if you otherwise grant permission by a separate written consent. In addition to training, we also have a service mission. WCU CMHS is dedicated to providing quality psychological services to the greater West Xxxxxxx community at low cost. Unfortunately, we are not able to provide all services and may have a waiting list depending upon the availability of student clinicians. If we cannot assist you, we will attempt to provide you with several referrals. ASSESSMENT SERVICES Providing a thorough assessment/evaluation will include direct, face-to-face contact, interviewing, testing and/or completing questionnaires. It may also include time required for the reading of records, consultations with other psychologists/professionals, scoring of tests, interpreting results, and any other activities to support these services. In order for the evaluation to provide valid results, it is important that you help as much as you can by providing full answers and making an honest effort. If you have questions or concerns about the assessment or any of the procedures, the student clinician is available to discuss them. Effective treatment depends upon a number of factors, including the establishment of a good student clinician-patient alliance, as well as active participation by the patient. For therapy to be most successful, you will have to work on the things we talk about both during the sessions and at home. Psychological treatment includes potential for some risk as well as benefits. Since therapy involves discussing unpleasant aspects of your life, you may experience uncomfortable feelings. The evaluator, supervisor and small team of clinical graduate students may view the tapes and discuss your case as part of their training. All such tapes will be erased as soon as possible after services have been rendered. The only exception is if you otherwise grant permission to a faculty supervisor by a separate written consent. Health Insurance Portability and Accountability Act (HIPAA) A new federal law, HIPAA, provides new privacy protections for medical records and new client rights with regard to the use and disclosure of your Protected Health Information (PHI) used for the purpose of treatment, payment and health care operations. HIPAA requires that we provide you with a Notice of Privacy Practices (the Notice) for use and disclosure of PHI for treatment, payment, and health care operations. The Notice, which is attached to this agreement, explains HIPAA and its application to your personal health information in greater detail. The law requires that we obtain your signature acknowledging that we have provided you with this information at the end of this session.
Purpose and Mission. The purpose of the Golden Essence Dance Team is to promote school spirit, involvement, teamwork, dedication, strong work ethic, and respect. A successful team is a result of two things: mutual respect among team members and a common vision about where the team is going. The mission of the Golden Essence Dance Team is to provide dancers with a successful and meaningful experience. Team members should have personal goals of high academic achievement, increasing their dance ability, learning effective time management, forming strong relationships, and learning to become a valuable team member. The success of the team is due to the incredible dedication from the dancers and advisor(s). Dancers should exhibit great sportsmanship, positive attitudes toward teammates, schools and other organizations. The dancers MUST have the utmost respect for their advisor(s) and captain/co-captain(s).
Purpose and Mission. The District and the Tribe desire to evaluate and improve the academic progress of Native students attending public school in the District and improve compliance with the Every Student Succeeds Act of 2016 (ESSA). The purpose of this data sharing Agreement is to work to close the opportunity gap, and increase the graduation level for our Native American students in the spirit of the Centennial Accord and Millennium Agreement with the State of Washington and as identified under ESSA. The reason for this Agreement is to allow the Parties to legally share data to conduct program evaluation as outlined in the Parties Memorandum of Understanding for Delivering Title VI and Other Services and Supporting Culturally Relevant Curricula. The agreement is entered into in the spirit of building community relationships and cultural understanding to improve the success of tribal students. This Agreement designates the Behavioral Health and Family/Youth Services Manager as an authorized representative of the Tribe to receive student data from the District consistent with FERPA so that the Tribe may receive student education records, including personally identifiable information, without written parental/guardian/eligible student consent under 25 C.F.R. § 43.14(g) and 34 C.F.R. §99.31(a)(3) in order to evaluate the academic achievement of all Native students attending school in the District. The Tribe maintains civil jurisdiction over matters of juvenile truancy for tribal citizens within external boundaries. If the Tribe implements a truancy program in the future the designated truancy officer in the tribe is permitted to use this data to address truancy.

The Gen-1 Theme House is a University of Cincinnati housing facility designed to provide its residents with a safe, orderly, and highly structured environment in which to live, learn, and work. The objective of the Gen-1 Theme House is to provide first-year, first-generation, full-time freshmen with the support needed to make a successful transition from high school to college. The Gen-1 Theme House Policies & Procedures and expectations are established to support the purpose and mission of the house and to maximize the opportunity for each student to achieve success in his/her first year of college.
