Protocol Requirements. To participate in the CDTM Program, the treating Physician for each Patient must establish, upon consultation with one or more Pharmacists, a written protocol specific to such Patient (“Patient Protocol”). The Physician shall approve and sign the Patient Protocol, send to the Pharmacy and a copy of each Patient Protocol shall be maintained as part of this Agreement as a separate Schedule P, the form of which is attached hereto. Each Patient Protocol must include, but is not limited to the following: The specific drug(s) to be managed by the Pharmacist; Terms and conditions under which such Patient’s drug therapy may be implemented, modified and/or discontinued; A list of lab tests that the Pharmacist may order; The conditions and events that require the Pharmacist to notify such Patient’s Physician(s); and The drugs that may be administered by the Pharmacist.
