Project Reporting Requirements. As provided in the attached Request for CDBG Reimbursement and Quarterly Performance Report (Quarterly Reports must be uploaded to Pasco County Community Development website via Neighborly Software): Direct benefit activities, those with a national objective of LMC (Low-Moderate Clientele), generally require the following accomplishment data: Number of beneficiaries Race/ethnicity of beneficiaries Income level of beneficiaries Leveraging funds Performance measures – what was accomplished A-4 Project Schedule: Request for CDBG Reimbursement and Quarterly Performance Reports are to be submitted every ninety (90) days; invoices are to be submitted on a monthly basis or no less than quarterly; and the receipt and use of Program Income shall be submitted on a quarterly basis. The Project shall be completed on or before XXXXXXXXXXX, and final invoices, performance reports, and accounting of Program Income are due no later than ten (10) days after the end of the Project Term. All reporting documentation required pursuant to the terms of this Agreement shall be uploaded to Neighborly. The SUBRECIPIENT is responsible for income qualifying clients via HUD standards and income ranges. Such income verification shall be on an income qualification form (Attached and incorporated herein as Exhibit C) and signed by the client. The completed income qualification forms shall be uploaded quarterly into Neighborly. Project Reporting Requirements. Beginning with Metro’s first disbursement of any portion of the Eligible Share to LIP for a Project, and continuing each year thereafter, on or before of each year during the Term until Unit Targets are completed and/or all Eligible Share is disbursed, LIP will provide a financial report to Metro containing the following information (a) an itemized list of LIP’s expenditure of Project Funds (and interest earnings thereon) through the end of the applicable calendar year and (b) information regarding progress or completion of any approved Project that was not previously reported as complete.
Project Reporting Requirements. Progress and Final Report(s): The Grantee shall provide quarterly progress reports and a final report as specified in the Project Schedule. Quarterly progress reports shall reflect work completed in the previous three months, and final reports shall reflect the entire Grant period. A progress report shall also be submitted for the time period immediately preceding the submission of the final report.
