Mayo Clinic Arizona 00000 Xxxx Xxxx Xxxxxxxxx Mayo Clinic Collaborative Research Building Scottsdale, AZ 85259 Information on Requesting Access to Mayo Colon Cancer Family Registry Data in dbGaP Dear Researcher: Data from the Mayo Colon Cancer Family Registry (Colon CFR) has been deposited in dbGaP with specific data use limitations that reflect the conditions of informed consent under which these specimens and data were collected. First, the consent form is clear that this data can be used only for research relevant to colon cancer. The consent states “In signing this consent, you do give permission for researchers to analyze and search for genes from your blood or tumor that are suspected of being related to colon cancer predisposition. No other types of genes will be studied.” Second, the data and specimens were entrusted to the Mayo Colon CFR investigators. These can be shared for research purposes but we must be responsible for reviewing any and all proposed studies to insure that studies conform to our consent processes. Third, in recognition of the responsibility and major effort that went into establishing the Mayo Colon CFR, we request that the Mayo investigators be considered as collaborators for the proposed project if this seems reasonable. SECURITY BEST PRACTICES Introduction The data sets provided in conjunction with this agreement are controlled access data. The procedures described below are based on the assumption that access to deidentified individual (person) level detailed genomic data associated with phenome data should be controlled and not publicly available. The goal of this process is to ensure that data provided by the NIH is kept sufficiently secure and not released to any person not permitted to access the data, either through malicious or inadvertent means. To accommodate these requirements, systems housing these data must not be directly accessible from the internet, and the data must not be posted on any web or ftp server. Data placed on shared systems must be secured and limited to those involved in the research for which the data has been requested. If data is stored on laptops, mobile devices or removable media, those devices must be encrypted and password protected. Protecting the Security of Controlled Data
