Private Contract. The COMPANY and MSRC agree that this Agreement is a private contract and should not be construed or interpreted to be a public contract or a private contract creating public duties.
Private Contract. Group understands that Blue Shield is prohibited by CMS from paying capitation to, or including in its network, any provider that has entered into a private contract with a Member for the provision of services. Blue Shield reserves the right to terminate any such provider from its network. Further, if the provider so terminated was a Group Physician, Blue Shield shall have the right to reduce Group’s capitation by the amount of any capitation that was paid either directly or indirectly to such provider(s). This provision shall remain in effective for a period of two (2) years from the time that all direct contracts between provider and Member have been terminated.
Private Contract. I, the Patient agree that this is a private contract between My Doc L.L.C. and Myself for medical services received. I agree that I am not a Medicare Beneficiary. I agree that Medicare cannot be billed for any services performed on my behalf by My Doc L.L.C. In addition, I agree not to xxxx Medicare or attempt Medicare reimbursement for any such services. By agreeing to the terms and conditions of this Agreement, I am solely responsible for all the fees for medical services provided by My Doc L.L.C. I am in complete understanding that My Doc L.L.C. makes no representations whatsoever that any fees paid under this Agreement are covered by my health insurance or third party plan applicable to me. I, the Patient shall retain full and complete responsibility for any such determination.
Private Contract. I understand that Xxxxx Xxxxxxx will hold me responsible in any one of the following situations. I may be asked to review and sign the Private Contract form in addition to this form:
Private Contract. The parties acknowledge that the Institution is not a party to this Agreement and that this Agreement is a private contract between Consultant and Company. Institution will exercise no authority or control over Consultant in his/her performance of his/her Consulting Services hereunder. As such, Consultant and Company agree that Institution and its affiliated entities have no liability or responsibility to either party under this Agreement.
Private Contract. This contract cannot be entered into by myself, the Medicare beneficiary, or by my legal representative during a time when I, the Medicare beneficiary, require emergency care services or urgent care services. (However, a physician/practitioner may furnish emergency or urgent care services to a Medicare beneficiary in accordance with §3044.28 of the Medicare Carriers Manual). I the Medicare beneficiary or my legal representative will receive or have received a copy (a photocopy is permissible) of this contract, before items or services are furnished to me under the terms of this contract. • Xxxxx X. Xxxxxxxxxx, M.D. is not excluded from participating in Medicare Part B under [1128] §1128, [1156] §1156, or [1892] §1892 of the Social Security Act. • Xxxxx X. Xxxxxxxxxx, M.D. will retain the original contract (original signatures of both parties required) for the duration of the opt-out period.
Private Contract. The Purchasing Party undertakes to deliver the amount of _ _ VAT included, upon signature of the private purchase agreement by any of the means aforementioned.
Private Contract. If Patient is eligible for Medicare, or during the term of this Agreement becomes eligible for Medicare, then Patient agrees to sign a Private Contract in the form designated by Practice. To the extent required by law, Patient agrees to enter into a renewed Private Contract every two (2) years, as requested by Practice. Practice will not respond to e-mails or other messages that contain sensitive medical information. If a response is requested, Practice will respond through the secure patient portal. Though it is Practice’s policy only to respond through the patient portal, by initiating correspondence through an unsecure and/or unencrypted channel, Patient hereby expressly waives Practice’s obligation to guarantee confidentiality with respect to correspondence using such means of communication. Patient understands and acknowledges that Practice may retain any communications between Practice and Patient and include such communications in Patient’s medical record.
Private Contract. The IFB is administered by the City of Xxxxxx and its Community Development Authority. However, the resulting contract will be between the Applicant and the contractor selected as the lowest responsive and responsible bidder. Even though funds are provided through the Authority, as the parties to the contract will be private persons, the contract shall not be deemed to be a public contract.
Private Contract. If Patient signs up for Medicare during the term of this Agreement, then Patient agrees that this Agreement will automatically terminate, unless Patient notifies Practice and Patient signs a Private Contract in the form designated by Practice. To the extent required by law, Patient agrees to enter into a renewed Private Contract every two (2) years, as requested by Practice.
