Privacy Act 1988. The Bonded Medical Program The Bonded Medical Program (the Program) is a statutory scheme which provides students with a Commonwealth Supported Place (CSP) in a medical course at an Australian university, in return for a commitment to work in regional, rural or remote areas for a specified period fulfilling their Return of Service Obligation (XxXX). Agreement to Participate Participants of the Program are bound by Part VD of the Health Insurance Act 1973 (the Act) and must abide by the Health Insurance (Bonded Medical Program) Rule 2020 (the Rule). Pursuant to subsection 124ZE(2) of the Act, you will become a Bonded Medical Program participant if: you were a party to a deed of agreement with the Commonwealth (as represented by the Department) for funding of a place in the Bonded Medical Places Scheme, including a deed of agreement as varied; and you advise the Department that you wish to opt in to the Bonded Medical Program under section 124ZU; and the Secretary agrees, in writing, to your participation in the Bonded Medical Program. This agreement serves as formal notice that you voluntarily wish to opt-in to the Program for the purposes of subsection 124ZU(2) of the Act. By signing this agreement, you are advising the Department that you wish to voluntarily opt in to the Program and that you voluntarily agree to participate in the Program using a web portal maintained by the Department: Full Name: Signature: Date: After returning this signed agreement to the Department, the Secretary will then decide whether to agree to your participation in the Program. If the Secretary agrees, you will receive an email welcoming you into the Program. The email is the Secretary’s written agreement to your participation in the Program, and you will become a bonded participant on the day after you receive that email.
