PREPAID HEALTH BENEFITS. 31.01 When the enrolment and other requirements of the insurer(s) as indicated in the contracts with the insurers have been met, the Employer shall implement the following group plans in accordance with the Alberta Health Services Standard Plan:
PREPAID HEALTH BENEFITS. (a) Eligible employees may elect to enroll in any or all of the group insurance plan(s) at the time of hire. Employees who have elected to enroll in a particular Plan may withdraw at any time. An employee who has enrolled in a Plan or has withdrawn may enroll in a Plan subject to Carrier approval and conditions. Re-enrolment shall occur only at the sign-up opportunities in January and July.
PREPAID HEALTH BENEFITS. Article 31 shall be amended as follows: The Parties agree that the actual coverage provided by either Insurance, Long- Term Disability, and/or Dental shall be governed by the terms of the plans that have applied to Home Care Employees, as attached. Such terms replace those provisions of the Health Care Agreement that provide for specific and particular types of coverage.
PREPAID HEALTH BENEFITS. 27.01 The Employer agrees to provide benefits to eligible Regular Full-Time and Regular Part-Time Employees of The Salvation Army Edmonton Xxxxx Xxxxx in accordance with the terms of the Employer’s Benefit Plan. Employees who regularly work twenty-five (25) hours per week or more shall participate in the benefit plan. Employees who work thirty (30) hours per week or more are eligible for long-term disability. As of the date of ratification, the Benefit Plan includes coverage for: • health care • dental care • long-term disability • life insurance • accidental death and dismemberment. The types of coverage and benefits in the Benefit Plan and the operation of the Benefit Plan shall be governed by the terms and conditions of the Benefit Plan and the contracts between The Salvation Army and the benefit insurers, as provided for in the Employer’s Policy. The Employer shall make information booklets available to eligible Employees who participate in the benefit plans.
PREPAID HEALTH BENEFITS. 21.01 The Employer shall provide a Health Benefits Plan for Regular and Temporary Employees, subject to the enrolment and other terms and conditions of the Insurer. All qualifying full-time and part-time Employees are required to enroll in mandatory insurance benefits. The Health and Dental benefits may be waived by the Employee with proof of coverage.
