Common use of Premium Rate Clause in Contracts

Premium Rate. Your Premium rate is determined using the following acceptable rating factors: age, tobacco use, family size, and geography. [Your][An Enrollee’s] tobacco use impacts the determination of [Your][the] Premium rate. Upon enrollment, You will attest to [Your][an Enrollee’s] tobacco use.
