Pre-Employment Drug Testing. Except for Seller’s personnel whose Services Buyer determines may not exceed thirty (30) days in dura- tion, Xxxxxx agrees each person assigned to perform Services hereunder shall be required to complete a drug test as set forth in Addendum A, entitled “Drug Free Workplace,” which is by this reference made a part of this Order. The drug test must have been completed within one year from the date the individ- ual begins performing Services for Buyer. If Seller’s personnel are extended beyond thirty (30) days, Seller shall perform a drug test upon receipt of request from Buyer.

Pre-Employment Drug Testing. 4.1 Individuals that receive a job offer from AMR must complete a post-offer / pre-placement drug test that is administered by an AMR-designated provider. AMR’s Human Resources Department should provide guidance to employment candidates regarding HR-designated test locations, documentation and process requirements.

Individuals with positive drug 4 testing results will not be hired and may not apply or be considered for employment 5 for six (6) months after a positive test result. 6 Any violation of this policy may result in discipline up to and including termination. 7 The only exception to this rule is for an employee possessing or using as directed a 8 drug which is prescribed for that employee by a physician or licensed practitioner 9 and which does not impair safe or efficient job performance. 11 When a manager or supervisor has reasonable suspicion that an employee has 12 used or is under the influence of a substance regulated by this policy, the manager 13 or supervisor should initiate an investigation in accordance with the procedures 14 identified in this Policy. 16 Extensive training for supervisors in recognizing symptoms of drug intoxication and 17 impairment will be updated periodically. 19 Grande Ronde Hospital will not engage in random alcohol or drug testing of the 20 general employee population. 22 All testing will be done by labs licensed by the State of Oregon. Employees may 23 choose a testing site from a list containing at least two (2) labs in addition to the 24 hospital lab. 26 All tests and results will be identified in such a way as to ensure employee 27 confidentiality. Strict attention shall be paid to adhering to chain of custody 28 procedures when collecting and labeling specimen(s). 30 A test result in excess of cut-off levels will be sent to the Medical Review Officer 31 (MRO) contracted by the Hospital and trained in interpretation of drug and alcohol 32 testing results. The MRO will contact the employee and offer to meet to discuss the 33 test results. The employee will have the opportunity to confirm or rebut the test
