Population Health. As such, CDPH is wishing to pre-qualify vendors in each of these public health content areas. Qualifications of individuals, institutions, or collaborations for each public health content area is further described below.
Population Health a. The PHP shall use claims, encounters and other publicly available data sources to identify potential COVID-19 outbreaks and clusters, as defined by the Department. The PHP shall notify the Department of any potential outbreaks and clusters within one (1) Business Day of identification.
b. No later than August 1, 2021, the PHP shall conduct outreach to Members under 18 years of age to encourage routine immunization and provide information on how to safely access routine immunizations.
c. The PHP shall develop and implement new health education programs to educate Members about symptoms, prevention guidance, and treatment for COVID-19.
Population Health. UMMS has a uniquely diverse and vulnerable patient population, presenting a critically important opportunity to identify early signs of disease that are easily accessible from the electronic health care record. Programs will be developed to reach out to patients with these risk factors via electronic consultation, nurse educators and care coordinators, and specialty services to recommend early interventions and to partner with the primary care community and patients to bend the curve of “pre-disease” towards health.
a. Examples include programs that identify pre-diabetes or out of control diabetes using lab values like high glucose and hemoglobin A1C to target health education and drug treatment. Similar approaches target early kidney disease by rising creatinine, and early high blood pressure in pregnancy, known to worsen pregnancy outcomes. Identification of rapidly rising weight allows opportunities to improve healthy living, target food desserts, and introduce new highly potent weight loss treatments, like the GLP1 agonists and SGLT-2 inhibitors.
b. As physicians and scientists engage with data-driven models of disease progression, it is critical that they incorporate fairness from the ground up by bringing together scholars of medicine, data science, and AI/ML with members of the very communities suffering from the burdens of race and history. Disparities in race, gender, and socioeconomic status impact healthcare systems in ways that are hard to identify, and yet they are often reproduced in biased AI/ML predictions because their training data incorporate biases arising from population health disparities.

Time Period and Types of Data That May Be Accessed: To comply with the minimum necessary standard, the Healthcare Provider or its Business Associate will limit its request to only the PHI needed for the particular Population Health project, and which is generated during the 36 months prior to the request. The Healthcare Provider or its Business Associate will comply with procedures approved by the Health Current Data Governance Council or the Health Current Board of Directors to implement the HIPAA minimum necessary standard.

For public health practitioners, improving population health involves understanding and optimizing the health of a population broadly defined by community.
