PLEASE BE AWARE. In the event a client terminates and/or is discharged from services, Beacon Pediatric Behavioral Health reserves the right to terminate the payment plan agreement and require that the entire balance be paid in full within one week of the client’s last appointment. Upon termination/discharge of services, you remain responsible for any balance due for services previously rendered, regardless of your child’s status as an active or inactive client. The Student and Parent do hereby affirm and acknowledge the inherent hazards and risks associated with participating in a recital or theatrical production. The Student and Parent understand that such productions may involve physical movement and dance, as well as the operation of lights and other equipment that could result in physical injury or even death. Therefore, the Student and Parent do hereby release, waive, discharge, and agree not to xxx Acting Academy for Kids, Inc. (“AA4K”), the theatrical facility, or any associated producer, including but not limited to The Gate, the South Orange County Community Theatre DBA Camino Real Playhouse and Capistrano Shakespeare Festival and Capistrano Acting Academy, Community Roots Academy, and/or any other academic or educational institution with which AA4K may partner with from time-to-time for a specific production (the “Theatrical Partners”). The Student and Parent expressly assume full responsibility for any risk of bodily injury, death, or property damage, now and forever, arising out of participation in any theatrical production or related operations, including rehearsal and backstage, and performances (“Theater Work”) whether foreseen or unforeseen, and whether caused by the negligence of AA4K, the Theatrical Partners, or otherwise.   • New Employee Orientation The Union will provide each agency personnel director with the names and addresses of up to two (2) authorized Union representatives per agency to receive notice of each formal orientation meeting held by the Department. The notice will be sent as soon as such meetings are scheduled (but not less than ten (10) days in advance) and will include date, time and location. Due to operational exigencies, agencies may schedule an orientation which will provide the Union with less than the requisite ten (10) days' notice; however the Union shall be notified as soon as possible after the scheduling of the orientation and the Union representative shall be released from duty. Agencies shall routinely schedule orientations in a manner that will allow for the ten (10) day advance notice to the Union. During the formal orientation, the Union will be permitted to give a twenty (20) minute presentation which may include an enrollment in supplemental Union benefits. The parties shall encourage employee attendance, although attendance shall not be mandatory if an employee objects to attending the presentation. In the event a formal orientation meeting is not held, or the Union is unable to attend the formal orientation because the designated Union representatives cannot be released under Article 4, the Employer shall allow the Union representative and the employee(s) to meet during duty hours at a mutually agreed upon time and location for twenty (20) minutes Employee participation in these meetings shall be encouraged although an employee shall not be required to attend such a meeting.

  • Employee Orientation Each and every person working for a contractor, including sub- contractors, will be given an orientation to familiarize them with the site safety program. Unless otherwise specified, each sub-contractor is responsible for the orientation of their workers.

  • NOTICE TO EMPLOYEES REGARDING THE SAFELY SURRENDERED BABY LAW The Contractor shall notify and provide to its employees, and shall require each Subcontractor to notify and provide to its employees, a fact sheet regarding the Safely Surrendered Baby Law, its implementation in Los Angeles County, and where and how to safely surrender a baby. The fact sheet is set forth in Exhibit I of this Contract and is also available on the Internet at xxx.xxxxxxxxxx.xxx for printing purposes.

  • Child Coverage Limited to Coverage Under One Employee If both spouses work for the State or another organization participating in the State’s Group Insurance Program, either spouse, but not both, may cover the eligible dependent children or grandchildren. This restriction also applies to two divorced, legally separated, or unmarried employees who share legal responsibility for their eligible dependent children or grandchildren.

  • Materials for Employee Choice Each year prior to open enrollment, the Appointing Authority will give eligible employees the information necessary to make open enrollment selections. Employees will be provided a statement of their current coverage each year of the contract.

  • New Employee Orientations The County shall provide the Union written notice of County-wide new employee orientations, whether in person or online, at least ten (10) business days prior to the orientation. The notice shall include time, date, and location of the orientation. Representatives of the Union shall be permitted to meet with the new employees for up to thirty (30) minutes during a portion of the orientation for which attendance is mandatory. The Union shall provide the County at least five (5) business days prior to the orientation any materials it would like the County to distribute to new employees at the orientation. If the Union staff are unavailable, the County shall grant release time for one (1) union xxxxxxx to attend the orientation pursuant to Section 6.2.1 The County shall make best efforts to provide the Union with the name, job title, department, work location, work, home and personal cellular telephone numbers, personal email addresses, and home addresses of all employees in the bargaining unit every 90 days, but no less than once every 120 days.
