Physician's Verification. At the Agency Head or designee's discretion, the employee may be required to provide a statement, from a physician, who has examined the employee or the member of the employee's immediate family, for all future illness. The physician's statement shall be signed by the physician or his/her designee. This requirement shall be in effect until such time as the employee has accrued a reasonable sick leave balance. However, if the Agency Head or designee finds mitigating or extenuating circumstances surrounding the employee's use of sick leave, then the physician's verification need not be required. Should the Agency Head or designee find it necessary to require the employee to provide the physician's verification for present or future illnesses, the order will be made in writing using the "Physician's Verification" form with a copy to the employee's personnel file. Those employees who have been required to provide a physician's verification will be considered for approval only if the physician's verification is provided within three (3) days after returning to work.
Physician's Verification. The supervisor may require that an absence due to illness of three (3) or more days be supported by a physician’s statement that the employee was sick or injured and that the employee was incapacitated from work for that period of absence.
Physician's Verification. In order to be eligible for a leave, the employee will be required to submit a physician's verification that a disabling condition exists due to the employment-related assignment.
Physician's Verification. The granting of medical leave will require a written statement from the doctor treating the employee outlining the nature of the disability and estimating the time when the employee will be able to return to employment.
Physician's Verification. If the Department Head has reasonable cause to believe that an employee is abusing sick leave, the Department Head may require a doctor's verification of sick leave use. Falsification of such evidence is deemed cause for disciplinary action.
Physician's Verification. Employees taking three (3) or more successive sick days shall provide a physician's verification of absence within five (5) days after returning to work. Such verification shall be directed to the attention of the employee's Bureau Commander.
Physician's Verification. The District may require a physician’s verification of illness if a unit member has been on Sick Leave for more than five days. Unit members returning to work after surgery or a lengthy illness or injury will provide a doctor’s release certifying the unit member is able to perform the usual and customary duties of his/her assignment.

A physician’s certification may be required as evidence of an employee or family illness as outlined in Section 11.4, or any personal injury that prevented the employee’s attendance at work for a period in excess of three (3) continuous days. In addition, a physician’s certification may be required for any non-FMLA qualifying sick leave usage when an employee’s sick leave accumulation falls below 50% of possible accumulation. Falsification of such evidence will be sufficient cause for disciplinary action.
Physician's Verification. A doctor's written verification may be required as evidence of an employee or family illness as outlined in Section 15.2 or any injury that prevented his/her attendance at work for a period in excess of three (3) continuous days. Falsification of such evidence will be sufficient cause for disciplinary action. The employee will sign a medical release if additional medical evidence is required by the Sheriff. In addition, proof of disabling illness may be required for any sick leave usage when an employee’s sick leave accumulation falls below 50% of possible accumulation during the first two (2) years of employment. After two (2) years of employment, an employee with a history of short-term sick leave usage may be required to provide a medical statement of illness (for self or family member) if their sick leave accumulation falls below 96 hours. Falsification of such evidence will be sufficient cause for disciplinary action.
