Physician Absence. From time to time, due to vacations, illness , or personal emergency, a Physician may be temporarily unavailable to provide the services referred to above in this paragraph one. In order to assist Patients in scheduling non-‐ urgent visits, MODERN MOBILE MEDICINE will notify Patients of any planned Physician absences as soon as the dates are confirmed. In the event of a Physician’s unplanned absences, Patients will be given the name and telephone number of an appropriate provider for the Patient to contact. Any treatment rendered by the substitute provider is not covered under this contract, but may be submitted to Patient’s health plan.

During such times, Patient calls to the Physician will be routed to a message indicating absence from duty and what time full service is expected to resume. If Patients have a concern which is urgent and the Physician is absent from duty, they should proceed to the nearest urgent or emergent care facility.

In the event of the DPC Provider’s unplanned absences, Patients will be given the name and telephone number of an appropriate provider for the Patient to contact. Any treatment rendered by the substitute provider is not covered under this contract, but may be submitted to Patient’s health plan.

In order to assist Patients in scheduling non-urgent visits, CHS will notify Patients of any planned Provider absences as soon as the dates are confirmed. Substitute providers may be available outside the employment of CHS. Any treatment rendered by the substitute provider is not covered under this contract, but may be submitted to Patient’s health plan.

During such times, Patient’s calls to the Physician, or to the Physician’s office, will be directed to a physician who is “covering” for the Physician during his absence. The practice will make every effort to arrange for coverage but cannot guarantee such coverage
