Common use of Physical/Occupational Therapy Clause in Contracts

Physical/Occupational Therapy. Physical and occupational therapy is covered only when:  a program is implemented to restore the highest level of independent functioning in the most timely manner possible;  physical or occupational therapy is received from a licensed physical or occupational therapist;  physical or occupational therapy is ordered by a doctor;  the therapy will result in significant, sustained measurable functional or anatomical improvement of your condition; and  such improvement will not diminish with the removal of the therapeutic agent or environment. Physical therapy and occupational therapy services provided for habilitative or rehabilitative purposes are covered at the same benefit level. Inpatient Medically necessary inpatient physical or occupational therapy is covered as a hospital service. See Section 8.0.
