PERSONNEL FOLDER. Employees have the right to review their individual personnel folder in the presence of a Human Resources Department staff member. Access shall be scheduled at the convenience of the employee and the Human Resources Department. When requested, copies of all materials to be included in personnel folders shall be provided to individual employees.
PERSONNEL FOLDER. No material placed in the employee's personnel folder, including any evaluation of the employee, shall be used to discipline the employee unless a true copy is offered the employee. Employees shall be given an opportunity to answer, in writing, any discipline- related evaluatory statement or any statement that the employee believes may reasonably lead to discipline. The answer shall be attached to the statement in the folder. Employees may from time to time review material compiled in their folder except pre-employment data. Any material or evaluatory statement upon which disciplinary action is based shall be removed from the employee's folder if the employee was not offered a copy.
PERSONNEL FOLDER. A. Unit members shall have the right, upon request, to review the contents of their personnel file in the presence of a representative of the Administrator with the exception of confidential material gathered prior to initial employment. The unit member may elect to have an additional witness of his or her own choosing. Any material resulting from any event or occurrence involving a unit member that is to be available to the Board of Education is to be reduced to writing and placed in such file. The unit member shall receive a copy of the material at the time it is placed in his/her file. No material shall be placed in a unit member’s personnel folder until and unless the unit member has affixed his/her signature to the material. Such signature indicates only that the unit member has seen the material. B. Unit members shall have the right to insert written explanation or response to material in their personnel file. Such explanation shall be delivered to the Administrator within ten (10) school days after the unit member has received a copy of the material referred to in paragraph A above. C. For each unit member there shall be only one official personnel folder maintained by the District. Such folder is subject to the conditions of paragraphs A and B above. This folder will be the exclusive repository for all official unit member evaluations and material relating to the unit member’s employment record. D. Materials that are inaccurate, misleading, or inappropriate shall be removed from the personnel folder. Disagreements over whether material is inaccurate, misleading, or inappropriate are subject to the grievance procedure and shall be raised in a timely fashion. E. Material must be placed in the personnel folder within thirty (30) days of the event or occurrence giving rise to the material or with thirty (30) days after the District knew or should have known o f such event or occurrence.
PERSONNEL FOLDER. 26.1 Only one personnel folder shall be kept for each member of the Police Department, kept by the Personnel Department Each member shall have the right to inspect his/her personnel folder on request to the Personnel Director or his/her designee. Upon receiving a request, the folder shall be made available at the Personnel Director’s or his/her designee’s convenience, but not more than 2 working days after the request is made. The file shall be inspected in presence of the Personnel Director or his/her designee and shall remain in the Personnel Department at all times. 26.2 A copy of any warning to an employee or any other document critical of an employee’s performance shall be given to the employee before being placed in the employee’s personnel folder. The employee may provide a written response to an appraisal, evaluation, warning or other document critical of the employee’s performance, which will be included in the folder, if desired by the member and will be retained with the document to which it is responsive as long as that document remains in the personnel folder.

All material in the personnel folder, excepting letters of recommendation, which will be removed in the presence of the teaching assistant, shall be open to the scrutiny of the teaching assistant. 2. The teaching assistant shall have the privilege and right to submit a written reply to any material in his/her personnel folder. His/her answer shall be reviewed by the Superintendent, then attached to the appropriate material and placed in the folder. In the event that the teaching assistant questions the factual accuracy of any material in the folder, the teaching assistant may submit proof supporting his or her position. The material will not be placed in the file if it is proven to be factually inaccurate. 3. No material in the teaching assistant’s personnel folder will be shown to anyone outside the District Administrators or to the Board, without the express written consent of the individual teaching assistant involved.
PERSONNEL FOLDER. A. Each teacher will have the right upon request and in the presence of the Superintendent of Schools or his designee to review and copy the contents of his complete personnel file, with the exception of confidential references and recommendations. A teacher will be entitled to have a representative of the ASSOCIATION accompany him during such review. B. The teacher will acknowledge the review of such material by affixing his signature to the copy filed with the express understanding that such signature in no way indicates agreement with the contents thereof. The teacher will also have the right to submit a written answer to such material and that answer shall be reviewed by the Superintendent and attached to the file copy.
PERSONNEL FOLDER. An employee shall within five (5) working days of written request to the Executive Director have an opportunity to review his/her personnel folder in the presence of an appropriate official of the Administrative Department to examine any criticism, commendation or any evaluation of his performance or conduct prepared by the Employer during the term of this Agreement. No derogatory complaint letter or report shall be placed in the unit member’s file without their knowledge. The unit member administrator shall be given a reasonable opportunity to discuss the matter with the complainants. No material derogatory to a unit member administrator’s conduct, service, character, or personality will be placed in his/her personnel file unless the unit member has an opportunity to review the material. The unit member will acknowledge that he/she has had the opportunity to review such material by affixing his/her signature to the copy to be filed with the expressed understanding that such signature in no way indicates agreement with the contents thereof. The unit member administrator will also have the right to submit a written answer to such material, and his/her answer shall be reviewed by the Superintendent of Schools and attached to the file copy. Unit members, individually or accompanied by an Association representative, will have the right, upon request, to review and make copies of the contents of their personnel files. No unit member administrator shall be shown confidential recommendations received from sources outside the school system or within the school system at the time of appointment, promotion or contemplated promotion.
PERSONNEL FOLDER. An employee shall have the opportunity to review his/her personal history file in the presence of the Superintendent or his designee upon fifteen (15) days notice, and to place in such a file a response.
PERSONNEL FOLDER. 14.1 Access - Access to the personnel file shall be granted upon prior arrangement with the Superintendent, during the business hours when the office is normally open, and shall be in the presence of the District Administrator in charge of the file or a person designated by that Administrator for that purpose. The employee shall not have access to confidential, employment materials or references and shall not remove anything from the file.
