Performance Outcome Measures. Grantee will adhere to the performance and outcome measure requirements documented in Attachment A-1.
Performance Outcome Measures. 1. CONTRACTOR shall maintain an ongoing performance outcomes monitoring program using information in its provider database, Concurrent Review database, Beneficiary satisfaction surveys, and documentation completed by providers, including but not limited to monitoring of claims utilization patterns, assessment and screening tools, direct peer review and medical record audits. CONTRACTOR shall include in the outcomes monitoring program items required or recommended by the DHCS, as communicated by ADMINISTRATOR, and specified in the Reports Paragraph of this Exhibit A to the Contract.
a. Objective 1: CONTRACTOR shall achieve, track and evaluate timeliness of access to benefit services for Beneficiaries and Network Providers calling the Access Line. Timeliness measurements will include, but are not limited to, percentage and number of abandoned member calls to be no more than five percent (5%) of total monthly member calls, percentage and number of member calls answered within thirty (30) seconds to be no less than eighty-five percent (85%) and track call volume, and timeframe for routine calls from point of authorization to provider appointment.
b. Objective 2: CONTRACTOR shall achieve, track and evaluate utilization trends from claims-based data for outpatient and inpatient network provider mental health claims, identifying quality of care concerns related to over- and underutilization patterns as established within clinical program guidelines. CONTRACTOR shall report on the number and outcomes for cases exceeding utilization criteria approved by ADMINISTRATOR and conduct clinical reviews including, but not limited to, provider-peer consultation, beneficiary rescreening, Level of Care (LOC) reassignment and transition and Medical Director review.
c. Objective 3: CONTRACTOR shall achieve, track and evaluate no less than ninety percent (90%) satisfactory Beneficiary survey results with the customer service provided on the twenty-four (24) hour-seven (7) days a week Access Line. Measurement of satisfaction shall be determined by, but not be limited to; overall satisfaction with informing Beneficiaries of benefit information, accessing services, brief screening for services, and providing referral processes.
2. ADMINISTRATOR may identify contracted Network Providers to be reviewed. CONTRACTOR shall initiate review within thirty (30) calendar days of notification from ADMINISTRATOR and shall advise contracted Network Providers of reason(s) for the review. CONTRAC...
Performance Outcome Measures. 1. CONTRACTOR agrees that annually the percentage of Clients who complete the program by making substantial progress on their treatment plan goals and remaining in the program until their release should exceed 75%.
2. CONTRACTOR agrees that annually 95% of Clients who are working with the post release case management team will link to ongoing services within ten (10) calendar days of release.
3. ▇▇▇▇▇▇▇▇▇▇ agrees to engage in follow-up communication with all Clients going through the program. Follow-up contacts will occur at ninety (90) calendar days post-discharge and will gather information on current Client needs (e.g., need for resources, further services) as well as tracking post-treatment recidivism.
4. CONTRACTOR shall have and post at the location where services are provided, written procedures for obtaining medical or psychiatric evaluation and emergency services.

Performance Outcome Measures. The proposed Contract will include the following Performance Outcome Measures which are consistent with the National Performance Indicators. The Contractor shall ensure that the services provided under this contract contribute to this goal indicated in this contract and meet the outcome measures under that goal. Contractor’s performance goal should lead to the program’s outcome measures. Should there be a change in federal, State and/or County policies/regulations, the County may amend these outcome measures via a contract amendment, as described in Terms and Conditions, Section 8.7. Commencing with the first month of services, the Contractor’s performance will be monitored quarterly in the outcome measures under the following program goal: Goal #4: Partnerships among supporters and providers of services to low-income people are achieved. • Expanding Opportunities through Community-Wide Partnerships. Outcome Standard: 70% - 100% Acceptable Under 70% Unacceptable The Contractor’s performance percentage/rate is calculated as follows: Contractor is expected to meet a performance outcome based on the number of partnerships projected for contract period versus the number of organizational partnerships achieved in reporting month. The Contractor’s performance rate will be determined quarterly by dividing the total number of partnerships that actually achieved the outcome measure quarterly by the total number of partnerships projected to achieve the outcome measure as provided in Contractor’s Projected Outcomes, Technical Exhibit 12. The Contractor’s performance rate may be adjusted if, upon review, no data is available to support the performance rate. The County will have zero tolerance for any date manipulation committed by the Contractor.
Performance Outcome Measures. 4.1 The Contractor shall gather the following data elements and record them in an electronic system specified by OBH. Data shall be recorded within one week after any change in competency restoration status has occurred.
a. Date competency is raised by the court b. Date that jail screenings are completed (mental health, substance use, and suicide risk)
c. To effectively measure behavior modification and psychiatric stabilization involving participants in this program, Critical incidents to be reported to OBH include:
i. Incidents of Self Harm

iii. Assault on Staff iiii. Assault on Inmate d. Date of jail mental health evaluation
Performance Outcome Measures. The Contract includes four (4) Performance Outcome Measures that will measure the Contractor’s performance related to the Case Management Services for AHH participants. These measures will evaluate the Contractor’s ability to enroll, place, and maintain DHS clients in permanent housing by providing intensive case management and linkages to supportive services. AHH seeks to improve the health care status of participants by improving access to primary and specialty care and thus reducing emergency and inpatient utilization. Should there be a change in federal, State and/or County policies/regulations or the County determines the need for change, the County may amend these Outcome Measures via a Contract amendment, as detailed in Contract Section 9.1, Alteration of Terms. These measures are as follows: Assist in establishing SSI/SSDI benefits or obtain employment training and/or opportunities for at least sixty percent (60%) of the permanently housed AHH participants. Place at least eighty percent (80%) of AHH participants into permanent housing within 60 days of voucher issuance. Once the homeless DHS clients are placed in affordable housing, Contractor shall maintain a minimum of seventy-five percent (75%) of the homeless DHS clients in affordable permanent housing for six (6) consecutive months. Retain a minimum of sixty percent (60%) of the homeless AHH participants in affordable permanent housing for twelve (12) consecutive months. Demonstrate a reduction in AHH participants’ emergency room visit usage and inpatient admissions and associated lengths of stay for seventy percent (70%) of the permanently housed AHH participants. DHS will verify healthcare utilization by conducting pre and post record review of client service utilization.

1 2. CONTRACTOR agrees that annually 95% of clients will link to ongoing services within 2 ten (10) days of release who are working with the post release case management team.
3 3. CONTRACTOR agrees to provide a annual recidivism rate study comparing the re-arrest 4 rates of clients who have gone through the program to the general population who are released from jail.

Performance Outcome Measures. The proposed Contract will include three (3) Performance Outcome Measures that will measure the Contractor’s performance related to the Housing Locator Consultant Services for GR Single Adults. Should there be a change in federal, State and/or County policies/regulations, the County may amend these Outcome Measures via a contract amendment, as detailed in Section 8.7, Changes and Amendments of Terms. These measures, as applied to housing locators consulting services are as follows: • Contractor shall maintain a complete and accurate landlord database that includes at minimum 100 monthly bona fide per Housing Locator Consultant, (i.e. true and verifiable addresses, not just rental information from newspaper classified advertisement; or mail-box flyers ads), accurate permanent housing listing ,and accurate rental housing units, to rent/lease to homeless GR Single Adults per SPA in Los Angeles County. • Contractor shall successfully respond within 48 hours on 95 percent of the County’s GRHCM Consultant Services inquiries, per month during the term of the contract. • The Housing Locator Consultants will be required to maintain a Monthly Activity Log, which will document day-to-day activities, e.g., contact with GRHCM, canvassing of affordable rental housing search, development and maintenance of the Excel Housing database, training and tools provided to GRHCM, etc.
Performance Outcome Measures. CONTRACTOR shall be required to submit measureable outcomes on an annual basis, as identified in the COUNTY DBH’s Policy and Procedure Guide (PPG)
