Performance Measurement and Reporting. The MA Organization shall measure performance under its MA plans using standard measures required by CMS, and report (at the organization level) its performance to CMS. The standard measures required by CMS during the term of this contract will be uniform data collection and reporting instruments, to include the Health Plan and Employer Data Information Set (HEDIS), Consumer Assessment of Health Plan Satisfaction (CAHPS) survey, and Health Outcomes Survey (HOS). These measures will address clinical areas, including effectiveness of care, enrollee perception of care and use of services; and non-clinical areas including access to and availability of services, appeals and grievances, and organizational characteristics. An annual report to the Board of Directors detailing the investment portfolio’s composition and performance will be provided. Segregation of Duties For the purpose of cash management and investment, Ulta’s policy is to ensure that the following internal controls are in place to maintain adequate segregation of duties: • The separation of transaction authority from accounting and record keeping. • The separation of wire transfer and investment transaction duties. Non-repetitive wire transfers must be approved by a party outside the investment management process (excluding overnight investments). • The separation of record keeping with the reconciliation of company transactions with those reported by the third party custodian. The Treasury staff shall have responsibility and authority over internal controls; however, controls may be removed only with the written consent of the Chief Financial Officer.

Utilization Review: (a) An MA Organization for an MA coordinated care plan must use written protocols for utilization review and policies and procedures must reflect current standards of medical practice in processing requests for initial or continued authorization of services and have in effect mechanisms to detect both underutilization and over utilization of services. [422.152(b)] (b) For MA regional preferred provider organizations (RPPOs) and MA local preferred provider organizations (PPOs) that are offered by an organization that is not licensed or organized under State law as an HMOs, if the MA Organization uses written protocols for utilization review, those policies and procedures must reflect current standards of medical practice in processing requests for initial or continued authorization of services and include mechanisms to evaluate utilization of services and to inform enrollees and providers of services of the results of the evaluation. Performance Measurement and Reporting. In order to measure the performance of the Liquidity and Core Portfolios, performance benchmarks will be established for all portfolios. The performance benchmarks for the Liquidity Portfolio will be the average total return for the Xxxxxxxx index of money market mutual funds and the total return earned by the Washington State Local Government Investment Pool. The performance benchmark for the Core Portfolio will be the market yield earned by the Xxxxxxx Xxxxx’x 1 to 4.99 year Government Bond Index or, if Xxxxxxx Xxxxx index is unavailable, then a comparable index. The performance benchmark for the Mortgage-Backed portfolio will be the rate of return of the Xxxxxxx Xxxxx 0-5 year Mortgage Master Index. Performance reports shall be presented to the Executive Finance Committee on a monthly basis, and annually an in-depth investment performance report shall be presented to the committee. Reports will include the following information: -Interest earnings -Perfomance vs. indices -Gains/losses -Composition of pool -Dealer performance
Performance Measurement and Reporting. The Contractor’s Contract Manager shall provide management information reports to the awarding Authority. These shall be provided at no additional charge and, unless agreed otherwise, are to be provided at three-monthly intervals. The first reports to be provided 3 months after commencement of the Framework Agreement. Each report should cover the period since the date of the previous management information report and shall include, without exception, the following details relating to the Framework Agreement: • Details of all Service Orders placed by the Client/awarding Authority to include: − Number of assignment requests received; − Number of Service Orders (assignments awarded to the Contractor); − Number of hours per assignment (in total and by employee); − Cost of assignment (in total and by employee or relevant pricing structure, showing any discounts applied); • A report, by Service Category (Lot), of Services delivered to each business area of the Client/awarding Authority i.e. any other Government Department, Agency or Non-Departmental public body; • A report showing Charges paid in the period by individual Client. • A report showing the total amount of Charges paid in the period by the Client/awarding Authority. • A report identifying any failures to provide Services in accordance with the relevant Service Order; and • Details of the number and nature of any complaints from the Client/awarding Authority. In addition to the above the primary Client, HMRC requires the following summary reports relating to use of the framework by the Client and any other awarding Authority: • Total number of assignment requests received; • Total number of Service Orders (assignments awarded); • Total cost of Service Orders The submission, receipt and acceptance of these reports shall not prejudice the rights or remedies of either party under the Framework. The frequency and format of the reports may be changed by the Client and/or Contractor by mutual agreement during the life of the Contract in order to meet the Clients requirements for Management Information.
