Overview and Purpose. The Board is responsible for a standards-based teacher/school counselor evaluation policy which conforms to the framework for evaluation of teachers/school counselor(s) as approved by the State Board of Education and aligns with the “Standards for the Teaching Profession and the Ohio Standards for School Counselors” as set forth in State law. The evaluation policy is intended to provide an evaluation model that is research-based, transparent, fair, and adaptable to the needs of the District, students, and teachers/school counselor(s). The Board believes in the importance of ongoing assessment and meaningful feedback as a powerful vehicle to support improved teaching performance and student growth and to xxxxxx professional growth of school counselor(s), as well as promotion and retention decisions for teachers/school counselor(s). The purpose of the evaluation is to improve instruction, counselor services, and to make a record of the teachers’/school counselor(s) performance. The Board and the Association agree that the following evaluation procedure will be utilized during the life of the agreement. The content of the evaluation is not grieveable; however, a bargaining unit member may grieve a violation of the evaluation procedure.

Overview and Purpose. This Contract is a statewide centralized contract to acquire Electronic Poll Book (E-Poll Book) Systems and related services, accessories, consumables, training, and Maintenance as specified herein for all Authorized Users eligible to purchase through this Contract. In March of 2019, NYS passed legislation to allow for Early Voting and the use of E-Poll Book Systems, and NYS intends to implement the E-Poll Book Systems for use at Early Voting locations as well as Election Day polling places in 2019 and future elections. This Contract will allow the individual New York State County Boards of Elections (CBOEs) to procure E-Poll Book Systems either through a competitive Request for Quotes (RFQ), or by submitting a Purchase Order. Each CBOE will be responsible for the purchase of their own Equipment, and the implementation of the System, including training and support. This Contract outlines the procedures and methods used by Authorized Users, OGS, and Contractors to provide Authorized Users with a method for procuring E-Poll Book Systems. All Contractors will have the same terms and conditions for the duration of the Contract thus providing a fair and level playing field across the E-Poll Book System marketplace. It is highly recommended that Authorized Users procure E-Poll Book Systems via a Request for Quotes (RFQ).
Overview and Purpose. This state term contract has been established for the purchase of Potable Water and Food Grade Ice (collectively referred to as Product(s)). Minimum purchases are in truckload (TL) increments of each product, with a minimum order of one (1) TL. Potable Water and Food Grade Ice purchases support the state’s response following the declaration of a ‘state of emergency’ in one or more of Florida’s counties. Notwithstanding the above, Customers of this contract may wish to place orders outside of these events.
Overview and Purpose. The Third Party Cooperative Arrangements will create and/or expand Community Based Work Experience (CBWE) and career exploration activities for students with the most significant disabilities through braided funding from the Department of Education (DOE), Division of Vocational Rehabilitation (DVR) and the School District. Competitive employment is the first and preferred outcome for transitioning youth with disabilities, including youth with complex and significant disabilities. Paid CBWE is being emphasized in these arrangements as an evidence- based indicator for student success in postsecondary employment and independent living. DVR funding will be for deliverable services provided by up to two (2) Full-Time-Equivalent (FTE) School District Employment Specialist (ES) positions that will provide Supported Employment (SE) services to VR Transition Students with an implemented SE Individualized Plan for Employment (IPE). Students shall receive assistance in developing appropriate work skills, attitudes, behaviors, and work tolerance needed to plan for and achieve successful post high school employment. This Arrangement allows DVR to supplement services provided by the School District to students who are applicants for VR services or students eligible for VR SE services and not on a waiting list as a result of Order of Selection (OOS) or students with an implemented SE IPE. The School District shall not use DVR funding support to supplant the current level of services provided to the students. Each ES shall provide at least six (6) VR Transition Students, with an implemented SE IPE, with a CBWE by the end of the school year. In addition, each ES shall assist VR SE IPE students by referring them to the local One-Stop for comparable services and benefits that are commensurate to the services that the student would otherwise receive from VR.

Overview and Purpose. The Nemours Foundation is the owner of “Navigating the Health Care System.” Navigating the Health Care System (Curriculum) is a four-module health literacy curriculum designed by Nemours Children’s Health System for use with high-school-aged adolescents in classroom and community settings. It is designed to prepare students to be responsible for managing their own health care as they transition into adulthood. Inspired by the Curriculum’s strong evaluation outcomes, Nemours is seeking expansion partners to help bring the Curriculum to adolescents across the nation. The Nemours National Office of Policy & Prevention will serve as the COMPANY’s primary point of contact related to the work of this MOU. COMPANY desires to utilize the NHTS health unit. This MOU describes the intention of COMPANY and Nemours to collaborate on implementation of the NTHCS. Specifically, this MOU:  Documents the collaboration between COMPANY and Nemours.  Identifies broad, agreed upon roles and goals between COMPANY and Nemours that will enable COMPANY, Nemours and the community to receive the greatest value in return for the investment of time and resources in this collaboration.  Enables COMPANY to receive the information and support it needs from Nemours to successfully use NTHCS.  Enables Nemours to receive the information and support it needs from COMPANY to successfully coordinate COMPANY’s use of NTHCS.
Overview and Purpose. This Agreement represents a Service Level Agreement (“SLA” or “Agreement”) between Office of Children & Family Services Human Services Call Center (OCFS-HSCC) and Office of Cannabis Mangement for the provisioning of call handling services required to support and sustain certain identified and agreed upon CUSTOMER AGENCY lines of business. This Agreement outlines the parameters of all services covered as they are mutually understood by the OCFS-HSCC and CUSTOMER AGENCY. This Agreement does not supersede current processes and procedures unless explicitly stated herein. The purpose of this Agreement is to provide consistent service support and delivery to the Customer(s) of CUSTOMER AGENCY by OCFS-HSCC. The objectives of this Agreement are to: • Establish service ownership, accountability, roles and/or responsibilities. • Provide a description of service provision to the migrating agency. This Agreement is aligned with the goals and objectives of the statewide Call Center Consolidation project, which is led by the office of Information Technology Services.
Overview and Purpose. During the Research Term, Lilly and ProQR will collaborate in a research and pre-clinical development program with the intended purpose of: (a) selecting, discovering, generating, or identifying Compounds that are Directed To Project Targets; (b) performing Research and other activities to optimize such Compounds as Hits; and (c) performing Research and other activities to optimize such Hits as Successful Candidates (the “Research Program”). The Research Program will consist of Projects for each Project Target selected as set forth in this Article 3 and will be conducted pursuant to Workplans, as further set forth below in Article 4. For avoidance of doubt, the Parties intend that the Research Program under this Agreement will be a continuation of the Research Program under the Original Agreement, and any activities conducted thereunder between the Effective Date and the A&R Effective Date shall be deemed to have been conducted under the Research Program under this Agreement.
