Outreach Materials. All outreach materials, including flyers, advertisements, public service announcement scripts, etc., prepared with XX xxxxx funds must include a statement acknowledging that the project is sponsored by the State of Florida, Department of Children and Families, Refugee Services Program, with grants from the U.S. Department of Health and Human Services, Office of Refugee Resettlement. The Provider shall submit all materials to the contract manager for review and approval thirty (30) calendar days prior to publication and dissemination.

Outreach Materials. The participant shall submit to CMS for approval copies of all press releases and outreach materials including brochures, letters to physicians and/or beneficiaries, and media advertisements. Any press releases or outreach materials that include project results or financial information, projections of payments under the program, projected values of program services, or projected or actual savings are subject to approval by CMS prior to any release.
Outreach Materials. All materials produced for public distribution shall be reviewed and approved by the Contract Owner and the State prior to distribution and shall include the NH DES logo and the following citation: “Funding for this project was provided in part by a Watershed Assistance Grant from the NH Department of Environmental Services with Clean Water Act Section 319 funds from the United States Environmental Protection Agency”.

Grantees shall develop outreach and informational materials that include written, auditory, and visual options, inclusive of an outreach video in Spanish that can be shared on social media and with community partners. A transcript of the video in English shall be provided to the State. The outreach video shall be translated into other languages as needed following identification of relevant Patient populations in the Vermont Community Organization’s catchment area. The outreach and informational materials shall inform Patients, Providers, Practices, and community partners about the implementation and benefits of the Act 48 Provider Funding Opportunity and the Immigrant Health Insurance Plan (begins July 1, 2022). Grantee’s outreach and informational materials shall include information on the confidentiality provisions applicable to all applications and records created pursuant to 33 V.S.A. § 1902a33 V.S.A. § 1902a in accordance with the requirements of Act 48 of 2021. Developed outreach and informational materials shall include an identified point of contact for the Vermont Community Organization with at least telephone and email information included and include the contact information for the Office of the Health Care Advocate.

The factsheet will emphasize the project’s goals, how the project improves connections to the City’s biking and walking network and public input opportunities. The factsheet will be accessible to screen readers and be produced in English and Spanish languages. The Consultant will translate factsheet content to Spanish and finalize the Spanish-language factsheet layout. The Consultant shall produce two (2) project outreach materials, including one (1) postcard and one (1) flier. The postcard will be mailed to stakeholders within the project area to inform them of the project and the open house event. The flier will be distributed to project area businesses and property owners. All outreach materials will be designed in the current City templates. The Consultant shall coordinate with the City for the printing and mailing of outreach materials and the City will be responsible for printing and mailing-related expenses. Outreach materials shall be available in both English and Spanish language. The Consultant will translate outreach materials to Spanish and finalize the layout of the Spanish-language materials. Outreach Materials. The Contractor shall prepare outreach materials that provide brief project information summaries (not to exceed three) for efficiently conveying key messages, lessons learned, and valuable water resource management information to watershed management practitioners including local, state and federal government representatives. Outreach materials shall be developed with the goal to effectively communicate key findings including discussion of relationships between watershed function, land use development and water resource impacts in low-order stream systems and larger down-gradient waters resources (e.g., lakes, coastal waters, aquifers, etc). The information summaries should be designed with accompanying graphics and tables to clearly convey water resource impacts associated with inadequately managed IC conversion and the potential quantitative benefits of feasible watershed restoration activities/strategies identified in this study.
