Nutrition Services. 1. A Nutrition Services Employee who works more than twenty (20) uninterrupted workdays in a higher classification shall receive, retroactive to the first workday, the regular rate of the higher classification on their current step. (Example: Step 3 server to middle school cook is paid Step 3 middle school cook rate.) A break of five (5) workdays or less shall not be considered an interruption in meeting the twenty (20) uninterrupted workday requirement.

Nutrition Services. In addition to any nutritional consultation available under your health insurance plan, upon your request, the Company will arrange for you to have a consultation with a qualified community-based nutritionist, regardless of medical necessity. The cost of consultations will be your responsibility to the extent that the visits are not covered by your health insurance plan.
Nutrition Services. In addition to any nutritional consultation available under your health insurance plan, upon your request, the Company will arrange for you to have an annual personal nutrition consultation with a qualified nutritionist, and, also upon your request, for ongoing nutritional consultations with the nutritionist throughout the year, regardless of medical necessity. Results of the annual nutrition consultation will be communicated to you and, upon your request, based on the results, the Company will arrange for your Physician to develop a personalized nutrition plan for you. The cost of consultations will be your responsibility to the extent that the visits are not covered by your health insurance plan.
Nutrition Services. In the event that a regular Nutrition Services Department permanent position increases due to program changes associated with the position, extra time shall be offered to the current permanent Nutrition Services employees at that location. When a position becomes vacant for up to thirty (30) minutes at a school site, the District may increase the minutes of a current permanent staff member unless determined not qualified by testing. If the vacant position is more than thirty (30) minutes duration the position shall be posted.

  • Surgery Services This plan covers surgery services to treat a disease or injury when: • the operation is not experimental or investigational, or cosmetic in nature; • the operation is being performed at the appropriate place of service; and • the physician is licensed to perform the surgery. Preauthorization may be required for certain surgical services. Reconstructive Surgery for a Functional Deformity or Impairment This plan covers reconstructive surgery and procedures when the services are performed to relieve pain, or to correct or improve bodily function that is impaired as a result of: • a birth defect; • an accidental injury; • a disease; or • a previous covered surgical procedure. Functional indications for surgical correction do not include psychological, psychiatric or emotional reasons. This plan covers the procedures listed below to treat functional impairments. • abdominal wall surgery including panniculectomy (other than an abdominoplasty); • blepharoplasty and ptosis repair; • gastric bypass or gastric banding; • nasal reconstruction and septorhinoplasty; • orthognathic surgery including mandibular and maxillary osteotomy; • reduction mammoplasty; • removal of breast implants; • removal or treatment of proliferative vascular lesions and hemangiomas; • treatment of varicose veins; or • gynecomastia. Preauthorization may be required for these services.

  • Telemedicine Services This plan covers clinically appropriate telemedicine services when the service is provided via remote access through an on-line service or other interactive audio and video telecommunications system in accordance with R.I. General Law § 27-81-1. Clinically appropriate telemedicine services may be obtained from a network provider, and from our designated telemedicine service provider. When you seek telemedicine services from our designated telemedicine service provider, the amount you pay is listed in the Summary of Medical Benefits. When you receive a covered healthcare service from a network provider via remote access, the amount you pay depends on the covered healthcare service you receive, as indicated in the Summary of Medical Benefits.   • Transportation Services i) In the event that transportation services for a student served by CONTRACTOR pursuant to an Individual Services Agreement are to be provided by a party other than CONTRACTOR or the LEA or its transportation providers, such services shall be reflected in a separate agreement signed by the parties hereto, and provided to the LEA and SELPA Director by the CONTRACTOR. Except as provided below, CONTRACTOR shall compensate the transportation provider directly for such services, and shall charge the LEA for such services at the actual and reasonable rates billed by the transportation provider, plus a ten percent (.10) administrative fee, unless a “flat rate” is provided in the transportation contract. In the event that the transportation provider notifies the LEA or SELPA Director that CONTRACTOR is more than 90 days behind in payment for transportation services, LEA shall have the right, in its sole and exclusive discretion, but not the obligation, to make payment for such services directly to the transportation provider, and to deduct such payments from any sums owed to CONTRACTOR pursuant to this Master Contract and any Individual Services Agreement between the parties. In the event that the LEA makes direct payment of the transportation provider’s charges, it shall be entitled to withhold both the transportation charges themselves and such additional amount as shall be reasonably necessary to compensate the LEA for the staff and other costs incurred in making direct payment of those charges. The remedies provided to the LEA pursuant to this Paragraph shall not be exclusive. CONTRACTOR shall not include transportation through the use of services or equipment owned, leased or contracted through the LEA unless expressly provided in the Individual Services Agreement for the student transported.
