Notification of Members. Within thirty (30) days after learning of the effective date of termination of this Agreement or disaffiliation of Practitioner, Health Plan shall attempt to notify impacted Members regarding such termination and its effective date. Health Plan shall establish procedures to assist Members in the transfer of care to another Affiliated Provider. Health Plan shall inform Members regarding the personal financial consequences of continuing care with a disaffiliated provider. Provider or Practitioner shall notify any Member seeking services after termination that Practitioner is no longer affiliated with Health Plan for the provision of Covered Services.

  • Admission of Members Other persons may be admitted as Members from time to time pursuant to the provisions of this Agreement.

  • Termination of Membership You may terminate your membership by giving us written notice or by withdrawing your minimum required membership share(s), if any, and closing all your accounts.
