Notifiable Incidents. The Contractor must report any Notifiable Incident that occurs in relation to the Contract to the safety regulator (SafeWork SA) and any other regulator as relevant to the nature of the incident and related statutory requirements. This may include The Office of the Technical Regulator in relation to incidents involving gas or electricity, the Australian Maritime Safety Authority (AMSA), or the South Australian Environmental Protection Authority (EPA). The Work Health and Safety Act 2012 (SA) defines a Notifiable Incident as: the death of a person; or a serious injury or illness of a person; or a dangerous incident.

  • Data Incidents Merchant must report all instances of a Data Incident (as defined in the American Express Merchant Operating Guide) immediately to ISO after discovery of the incident.
