Notices of Change. These documents make up the entire agreement that is issued to the Enrolling Group. Policy Charge - the sum of the Premiums for all Subscribers and Enrolled Dependents enrolled under the Policy. When Preventive Care Medications are received from a non-Network Pharmacy, Benefits will be available to at least 80% of Eligible Expenses.
