Xxxxxxxx, room 237. I must have a clerk sign off on my discipline record and progress report from guidance. Parent or guardian must sign and agree to the terms. I must have the approval signature from my administrator to become an applicant for the 2018 Prom court. I must include an OFHS school appropriate picture no larger than 4X6. I understand that the picture I submit will be on display for all students to see. Pictures will not be returned. All applicants must agree to the following terms to be eligible: - Any tie in the votes will be awarded to the applicant with the highest GPA If I am selected as a 2018 Prom Court Applicant, I fully understand and agree to the terms listed above.
