NHS Outcomes Framework Domains & Indicators. 3.2 Local defined outcomes • Patients who meet NICE guidelines with symptoms from 12 weeks will be referred via their GP to their local Place based long COVID assessment and treatment service after underlying conditions have been investigated and excluded. • Each Place based long COVID assessment MDT will be equally accessible to everyone within that area and should consider health inequalities in the planning and delivery of the service. • Patients should be assessed holistically, including face to face where appropriate, and referred to relevant rehabilitation and therapy services to support their recovery locally. • Patients can receive ongoing support and treatment for persistent or worsening symptoms from the Cheshire & Merseyside regional Tier 4 MDT which will be led by LUHFT. • Local PR Teams have been resourced separately to create capacity to Domain 1 Preventing people from dying prematurely Y Domain 2 Enhancing quality of life for people with long-term conditions Y Domain 3 Helping people to recover from episodes of ill-health or following injury Y Domain 4 Ensuring people have a positive experience of care Y Domain 5 Treating and caring for people in safe environment and protecting them from avoidable harm Y support each of the Place based MDTs and submit data capture for the long COVID assessment service; similarly LHCH Clinical Health Psychology services have been resourced separately to provide psychology contributions to the care pathway and to support the MDT.
