Miscellaneous Reports. Upon the foreclosure sale of any Mortgaged Property or the acquisition thereof by the Purchaser pursuant to a deed-in-lieu of foreclosure, Countrywide shall submit to the Purchaser a liquidation report with respect to such Mortgaged Property, which report may be included with any other reports prepared by Countrywide and delivered to the Purchaser pursuant to the terms and conditions of this Agreement. With respect to any REO Property, and upon the request of the Purchaser, Countrywide shall furnish to the Purchaser a statement describing Countrywide's efforts during the previous month in connection with the sale of such REO Property, including any rental of such REO Property incidental to the sale thereof and an operating statement. Countrywide shall also provide the Purchaser with such information concerning the Mortgage Loans as is necessary for the Purchaser to prepare its federal income tax return and as the Purchaser may reasonably request from time to time. The Purchaser agrees to pay for all reasonable out-of-pocket expenses incurred by Countrywide in connection with complying with any request made by the Purchaser hereunder if such information is not customarily provided by Countrywide in the ordinary course of servicing mortgage loans similar to the Mortgage Loans.

To ensure CIRTS data integrity, the following timeframes are required for entering data into CIRTS: CIRTS Enrollment Screen reflects ACTV – Within 10 working days CIRTS Enrollment Screen reflects appropriate termination code no later than 30 days after services ceased. Assessments – Within 30 days of Assessment Date Care Plans – Within 30 days of the Care Plan Date Received Services – For those services allowing monthly aggregate reporting with zero unit entry required annually, the Provider must upon enrollment or first actual date of services, but no later than 30 days after ACTV enrollment date, complete the zero unit entry. Failure to ensure the collection and maintenance of the CIRTS data may result in the Agency enacting the Financial Consequences of Non-Performance, clause in Section 38 of this Agreement or the Termination clause in Section 51 of this Agreement, including delaying or withholding payment until the problem is corrected. CIRTS Data Entries for Providers The Provider will enter all required data for clients and services in the CIRTS database per the DOEA Program and Services Handbook and the CIRTS User Manual – Aging Provider Network users (located in Documents on the CIRTS Enterprise Application Services). Data will be entered into CIRTS before the Provider submits their request for payment and expenditure reports to the Agency. CIRTS data for services received must be entered into CIRTS by the 10th day of the month subsequent to the month in which the services were delivered. Services entered after this date will not be reimbursed. When a client’s services are terminated, the Provider must ensure that all invoices are received from subcontractors and/or vendors no later than 30 days after services stopped. Once entered into CIRTS, received services cannot be changed from one DOEA funding source to another DOEA funding source. Provider Monthly CIRTS Reports The Provider will run monthly CIRTS reports and verify that client and service data in CIRTS is accurate. This report will verify client information provided by the Provider and will be reviewed by the Agency before the Provider’s request for payment and expenditure reports can be approved by the Agency.
