Membership Lists. Clubs will submit a list of Home Club Members each year along with the Club Renewal Application/Worksheet and remit by the deadline shown, so that CFDA is able to fulfill its obligations to the Insurance Company. All clubs whether participating in the CFDA Club Liability Insurance Program or not MUST still report a membership list, according to CFDA rules. CFDA must provide membership numbers to our insurer for both our CFDA Affiliated Club Program and our CFDA Master Policy. Please use real names and aliases (if available) on membership lists. CFDA will verify that club members are members in good standing of CFDA and provide renewal dates, when the signed Agreement is sent back to the club.

Membership Lists a) The Employer agrees to provide the Association with limited access to the Human Resources Information System (HRIS) for part-time faculty members. The information available to the Association shall include: full name, sex, Canadian citizenship or immigration status, employee number, Seniority, home address and telephone number, University e- mail address where available, total salary and stipends of part-time faculty member for the preceding academic year, and effective date of first part-time contract. b) The Employer will ensure that the Association's offices are connected to the University's Human Resources Information System (HRIS) and will provide the Association with the software, training and technical assistance necessary to access the HRIS. In the interim, the Employer shall continue to provide a hard copy of the data.
