Medication Management Services. The Contractor shall allow PCPs to provide medication management services (prescriptions, medication monitoring visits, laboratory and other diagnostic tests necessary for diagnosis and treatment of behavioral disorders) to members with diagnoses of depression, anxiety and Attention Deficit Hyperactivity Disorder (ADHD). The Contractor shall make available, on the Contractor’s formulary, medications for the treatment of these disorders. The Contractor is responsible for these services both in the prospective and prior period coverage timeframes.

Medication Management Services. Your signature upon the Authorization for Client Consent for Treatment Form indicates consent for treatment by the family mental health nurse practitioner to provide therapy and medication management. Written consent need only be obtained one time for all subsequent care given by the nurse practitioner to the client in this office, though this needs to be updated annually. A minor under 18 and adults under guardianship must have a legal guardian in attendance for each appointment unless other arrangements have been made with your provider. No medication will be initiated or changed without the consent and knowledge of the legal guardian.
Medication Management Services. 50.1. The parties to the Agreement, together with the Society, acknowledge that there are benefits in pharmacists working cooperatively with general practitioners and other members of the healthcare team to address medication related needs of members of the community. Community pharmacists’ contribution to patient care is based on their specialised knowledge and expertise, and includes the provision of Medication Management Services (MMS) in residential aged care and domiciliary settings.

Medication Management Services. CONTRACTOR shall provide Medication 20 Management Services which include evaluation of need for medication; evaluation of clinical 21 effectiveness and side effects of medication; obtaining informed consent; medication education 22 including discussing risks, benefits, and alternatives with the Consumers or significant support persons; 23 plan development related to the delivery of services and/or to the status of the Consumer’s community 24 functioning; and prescribing, dispensing, and administering of psychiatric medications. Medication 25 Management Services shall be provided by a licensed physician with a specialty in psychiatry or by a 26 licensed RN, NP, or an LPT under the supervision of a licensed psychiatrist, and in accordance with 27 their scope of practice, CONTRACTOR is encouraged to follow COUNTY’s prescribing guidelines.

  • Project Management Services Contractor shall provide business analysis and project management services necessary to ensure technical projects successfully meet the objectives for which they were undertaken. Following are characteristics of this Service:
