MEDICAL POLICIES. 1. We agree to provide PHCA with up-to-date health records, written proof of medical exams (when applicable).

MEDICAL POLICIES. Any of the Company’s right, title and interest in, to and under insurance policies covering or relating to medical malpractice;
MEDICAL POLICIES. 7. For children’s comfort and to reduce the risk of contagion, parents/guardians must pick up their child 1 hour after notification. Children absent due to reportable disease may return only with a physician’s note indicating that the child is no longer contagious.
MEDICAL POLICIES. A. The Player agrees to: (1) report in good physical condition at the time and place fixed by the Club, and (2) keep himself in good physical condition during the entire season. Player further agrees not ingest either orally, by injection or otherwise, any narcotic, habit-forming, illegal or hallucinatory drug except when prescribed by the Club's physician for medically-related illnesses or medical emergency. To monitor this provision, the CBA may administer urinalysis tests to the Player at any time, and without notice, during the term of this Agreement.
MEDICAL POLICIES. A. Total contribution to the health insurance program will be based on the annual negotiated renewal provided to the DISTRICT by the health insurance program brokers in the spring of each year for the following year's rates.
MEDICAL POLICIES. A. As appropriate, the Team may direct the Player to undergo medical examinations and treatment, from a health care provider selected and provided by the Team. The Team pays for these examinations and treatments. The health care provider may furnish all information pertaining to the Player to any health care provider designated by the Team.
MEDICAL POLICIES. Future Com shall maintain in effect the medical insurance policies set forth on Schedule 2.3.3.2 for the employees identified on that Schedule; provided however, that Xxxxxxxxx or Future Com may substitute other medical policies with substantially the same coverages and benefits as the policies set forth on Schedule 2.3.3.2.
MEDICAL POLICIES. The Parties shall cooperate in an attempt to have PlayStream assign, and the Buyer assume, as of the effective date of the Employee Transition Date, the health insurance policy of PlayStream (or a new policy with the same provider and substantially the same terms).

The Team pays for such examinations and treatments. The health care provider shall furnish all relevant information pertaining to the Player to any independent health care provider designated by the Team.
