Medical and Insurance Benefits. 13.1 Employees will be included in the same Public Employees Benefit Board authorized state- employee benefits and the same wellness plans as analogous employees at UW Medical Center - Montlake. No other benefit plans shall be offered to Employees.

Medical and Insurance Benefits. 8.1 The City agrees to provide long term disability insurance with a 90-day waiting period and $50,000 life insurance to all full-time employees at no cost. The City also provides Employee Assistance Program coverage at no cost. Employees will also have the option to purchase additional life insurance and to provide coverage for their eligible dependents. Employees and their eligible dependents shall be offered the opportunity to enroll in medical/vision, and dental, insurance coverage. The employer also agrees to notify the Guild of any new insurance or other benefits that become available to the City during the life of this Agreement.

Medical and Insurance Benefits. 14.1 Medical, Dental and Vision Insurance. Regular full-time employees and regular part- time employees are eligible for medical, dental and vision insurance benefits. Any increases in premiums, if any, after Plan Year 2017, shall be borne equally by the Employer and the employee through payroll deduction. In no event shall employees’ premiums increase by more than 2% in each year for the employee-only level of insurance, and 4% for each other level of insurance and dependent coverage. In the event that a premium decreases, 50% of the decrease shall be allocated to the employee’s share of the premium. Participation in medical and dental benefits shall be subject to the enrollment and eligibility requirements of the specific plan. See Addendum A for premium contributions.

Medical and Insurance Benefits. 13.1 Medical and Dental Insurance - Beginning the first of the month following sixty (60) days of continuous employment, all full-time and part-time employees regularly scheduled to work twenty- four (24) or more hours per week shall be included under and covered by the Employer’s basic group insurance plan providing medical, surgical, hospital and dental insurance benefits at the current level of benefits. Participation in medical, dental and any other insurance benefits shall be subject to the plan’s specific eligibility requirements.
